摘 要:目的了解杭州市儿童猩红热患者的病原学特征,分析猩红热相关血链球菌毒力基因分布。方法采集杭州市儿童医院2016年4月至2018年6月32例猩红热患者咽拭子以及杭州市某幼儿园215份健康儿童咽拭子,分离培养,鉴定菌种,PCR检测化脓链球菌的emm基因,并进行分型。扩增分离自猩红热患者的缓症链球菌群血链球菌S28的毒力基因。结果从猩红热患者咽拭子分离链球菌22株,其中GAS占31.82%(7/22),缓症链球菌群占68.18%(15/22)。从健康儿童咽拭子分离链球菌36株,GAS占11.11%(4/36),缓症链球菌群占88.89%(32/36)。从S28中克隆到lmb、tig、gapA、psaA、fbp54、mf2共6个毒力基因,均为化脓链球菌的毒力基因。结论杭州市临床诊断的儿童猩红热21.88%(7/32)是由化脓链球菌引起,46.88%(15/32)是由缓症链球菌群引起的猩红热样病症,从这些患者分离的血链球菌中发现了与化脓链球菌致病有关的毒力基因。To investigate the etiology characteristics of children scarlet fever and virulence gene distribution of S.sanguis related to scarlet fever in Hangzhou.A total of 32 clinical throat swabs collected from children with scarlet fever in Hangzhou children’s hospital From April 2016 to June 2018 and 215 healthy children’s throat swabs from a kindergarten in Hangzhou were cultured,isolated and identified.The emm genes of S.pyogenes were detected by PCR and then classified.The virulence genes of S.sanguis S28 which was isolated from a scarlet fever patient was cloned.22 strains of Streptococcus were isolated from throat swabs of scarlet fever patients,of which S.pyogenes accounted for 31.82%(7/22)and Mitis Group accounted for 68.18%(15/22).36 strains of Streptococcus were isolated from healthy children’s throat swabs,with S.pyogenes accounting for 11.11%(4/36)and Mitis Group accounting for 88.89%(32/36).Six virulence genes:lmb,tig,gapA,psaA,fbp54 and mf2 were cloned from S28,which also appeared in S.pyogenes.Clinical children scarlet fever in Hangzhou are both caused by S.pyogenes as 21.88%(7/32)and Mitis Group which cause the scarlet fever-like disease as 46.88%(15/32).Virulence genes in Streptococcus sanguis isolated from scarlet fever which were also found in S.pyogenes.
