摘 要:目的探讨新生儿机械通气治疗中营养支持的应用价值。方法选取2018年7月~2019年6月在我院接受机械通气治疗的158例新生儿作为主要研究对象,运用随机数字表法将其分为两组,对照组(n=79)采用全静脉营养治疗,观察组(n=79)患儿采用早期微量喂养治疗,比较两组患儿的临床治疗效果。结果两组患儿的机械通气时间、体质量恢复时间比较,差异无统计学意义(P>0.05);观察组患儿的喂养耐受率高于对照组,差异有统计学意义(P<0.05);两种营养支持方法的不良反应发生率,两组患儿发生腹胀和胃潴留的比例比较,差异无统计学意义(P>0.05),观察组患儿的胃肠功能紊乱发生率和坏死性小肠结肠炎发生率显著低于对照组,差异有统计学意义(P<0.05);观察组患儿的达全量胃肠营养时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。结论在新生儿机械通气治疗中应用早期微量喂养的营养支持方法可获得理想效果,可提高患儿的喂养耐受性,且不良反应发生率低,同时可缩短患儿的达全量胃肠营养时间和住院时间,值得进一步推广和应用。Objective To explore and analyze the application value of nutritional support in neonatal mechanical ventilation therapy.Methods 158 neonates who underwent mechanical ventilation in our hospital from July 2018 to June 2019 were selected as the main subjects.The patients were divided into two groups according to the digital random table method.The control group(n=79)was treated with total intravenous nutrition,and the observation group(n=79)was treated with early micro-feeding.The clinical effects of the two groups were compared.Results There was no significant difference in mechanical ventilation time and body mass recovery time between the two groups(P>0.05).The feeding tolerance rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).By comparing the incidence of adverse reactions between the two nutritional support methods,we found that there was no significant difference in the incidence of abdominal distension and gastric retention in the two groups(P>0.05).The incidence of gastrointestinal dysfunction and necrotizing enterocolitis in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The time of reaching peak of gastrointestinal nutrition and hospitalization time of the observation group were shorter than the control group,and the differences were statistically significant(P<0.05).Conclusion The application of early micro-feeding nutrition support method in neonatal mechanical ventilation therapy can achieve ideal results,improve the feeding tolerance of children,and has a low incidence of adverse reactions.At the same time,it can shorten the time of reaching peak of gastrointestinal nutrition and hospital stay of children,which deserves further promotion and application.
