基于磁共振成像检查的青年人晶状体调节研究

摘  要:目的通过磁共振成像(MRI)检查观察青年人晶状体调节前后形态学变化,探讨相关调节机制。方法采用MRI获取眼球调节前后晶状体最佳横断面图片,经自动计算机辅助设计软件(Autocad 2010)处理,测量眼球调节放松时和调节紧张时前房深度、晶状体厚度、晶状体直径、玻璃体腔径、眼轴、晶状体前后弧长、前后曲率半径。根据数学计算获得晶状体前后弧度、前后横截面积及总面积,并做1.5T和3.0T MRI的比较试验。结果与调节放松时比较,调节紧张时前房深度变浅,晶状体厚度变厚,晶状体直径、晶状体前后曲率半径缩短,晶状体前弧长、前横截面积和总面积增加,晶状体前后弧度增大,差异均有统计学意义(均P<0.05);而调节前后玻璃体腔径、眼轴、晶状体后弧长和后横截面积均无明显改变,差异均无统计学意义(均P>0.05)。调节前后1.5T和3.0T MRI测量数据比较差异均无统计学意义(均P>0.05)。结论MRI能清晰获取晶状体在调节状态下的形态变化,明确人眼调节机制对老视、近视的治疗,人眼调节的重建,人工晶状体的研制具有重要的意义。Objective To observe the high-resolution imaging of the human eye lens with accommodation at rest and during accommodation by magnetic resonance imaging(MRI).Methods Eye MRI examination was performed in 30 healthy subjects(60 eyes)aged 20-24 years.The MRI data of the anterior chamber depth,the lens thickness,the lens diameter,the vitreous length and the axial length on 1.5t and 3.0t MRI scanner were collected when the eyes with accommodation at rest and during accommodation.The optimal cross-sectional images were selected and measured by computer aided design software(Autocad 2010).The anterior chamber depth,the lens thickness,the lens diameter,the anterior arc length,the posterior arc length,the anterior curvature radius,the vitreous length and axial length were measured when the eyes in relax and in accommodation.The anterior portions,posterior portions and the total lens cross-sectional area(CSA)were calculated.Results Compared with accommodation at rest,the anterior chamber depth became shallow,the lens thickness became thicker,the lens diameter,the anterior and posterior curvature radius of the lens were shortened,the anterior arc length,the anterior cross-sectional area and the total area of the lens increased,and the anterior and posterior arc of the lens increased,with statistically significant differences(all P<0.05).There was no statistically significant difference in the vitreous cavity diameter,the eye axis,the posterior arc length and the posterior cross-sectional area of the lens between those with accommodation at rest and during accommodation(all P>0.05).There was no statistically significant difference between 1.5t and 3.0t MRI measurement data of the lens with accommodation at rest and during accommodation(all P>0.05).Conclusion The morphological changes of the lens with accommodation at rest and during accommodation can be obtain clearly by MRI,which clarifies the mechanism of eye accommodation and is of value in the treatment of presbyopia,myopia,reconstruction of human eyes and the developmen
