肛周脓肿微创置管连续冲洗加负压引流的临床分析

摘　　要：目的:探讨肛周脓肿微创置管连续冲洗加负压引流的临床效果.方法:将2017年7月-2019年7月在笔者所在医院肛肠科治疗的60例肛周脓肿患者随机分为两组,对照组使用传统切开高位挂线技术,观察组使用微创置管连续冲洗加负压引流,比较两组的疗效、各项治疗指标、肛瘘发生率及脓肿复发率.结果:观察组治疗有效率为96.67%,明显高于对照组的76.67%(P<0.05);观察组术后VAS疼痛评分明显低于对照组,愈合时间、住院时间均明显短于对照组(P<0.05);观察组肛瘘发生率及脓肿复发率低于对照组(P<0.05).结论:肛周脓肿微创置管连续冲洗加负压引流的临床效果显著,能减少手术创伤,避免破坏肛门括约肌自制功能,提升愈合效果,减轻患者痛苦,具有积极的临床意义.Objective:To explore the clinical effect of continuous washing and negative pressure drainage of perianal abscess.Method:A total of 60 patients with perianal abscess treated in our hospital from July 2017 to July 2019 were randomly divided into two groups.The control group was treated with traditional incision high thread hanging technology,and the observation group was treated with minimally invasive continuous tube washing and negative pressure drainage.The curative effect,treatment indexes,incidence of anal fistula and recurrence rate of abscess were compared between the two groups.Result:The effective rate in the observation group was 96.67%,which was significantly higher than that in the control group(P<0.05).The postoperative VAS pain score was lower in the observation group than that in the control group,the healing time and hospital stay in the observation group were significantly shorter than those in the control group(P<0.05).The incidence of anal fistula and recurrence rate of abscess in the observation group were lower than that in the control group(P<0.05).Conclusion:Minimally invasive catheterization for perianal abscess with continuous irrigation and negative pressure drainage has a significant clinical effect,which can reduce surgical trauma,avoid the destruction of anal sphincter self-made function,improve the healing effect,and reduce the pain of patients,which has a positive clinical significance.
