经脐单孔腹腔镜与传统腹腔镜手术在输卵管妊娠治疗中的应用

摘　　要：目的:比较输卵管妊娠患者行保留输卵管的经脐单孔腹腔镜手术与传统腹腔镜手术治疗效果,总结经验.方法:回顾性分析宁夏回族自治区人民医院妇科2018年期间收治的158例输卵管妊娠患者的相关资料,总结经脐单孔腹腔镜手术在妇科手术应用中的经验.结果:试验组患者术后24 h口服镇痛药频次与术后戳卡(trocar)切口愈合患者满意例数优于对照组(P<0.05),试验组患者手术时间与对照组相比明显延长,差异有统计学意义(P<0.05);两组患者术中失血量、术后24 h肛门排气例数、术后血hCG值下降至正常范围时间及住院时间无明显差异(P>0.05).结论:经脐单孔腹腔镜手术操因其在传统腹腔镜基础上具有术后患者疼痛感更轻、术后戳卡穿刺切口愈合美容效果更佳、术中标本完整取出更加容易等优势,是传统腹腔镜手术的有益补充.Objective:To compare the effects of trans umbilical single-port laparoscopic surgery and traditional laparoscopic surgery in patients with tubal pregnancy in Ningxia,summarizing relevant experiences.Method:A retrospective analysis of 158 cases of tubal pregnancy patients admitted to the Department of Gynecology of Ningxia Hui Autonomous Region People’s Hospital during the period of 2018.The experience of single-hole laparoscopic surgery in gynecological surgery has laid the foundation for the wide application of umbilical single-hole laparoscopic surgery in various surgical operations.Result:The number of patients with oral analgesics and postoperative trocar incision healing in the experimental group after 24 h was better than that in the control group(P<0.05).The operation time and control in the experimental group were compared,the difference was statistically significant(P<0.05).There was no obvious difference between the blood loss during operation,the number of anal exhausts at 24 hours after surgery,the time of-hCG reduction in post-operative down into the normal level and hospitalization time(P>0.05).Conclusion:According to the results of this study,trans umbilical single-port laparoscopic surgery is a useful supplement to traditional laparoscopic surgery,because of its less postoperative pain,the cure of puncture point healing much better and the intraoperative specimen is easier to take out.
