人工关节置换术中使用碘伏浸泡术区对肾脏功能的影响

摘　　要：目的:评价在人工关节置换术中使用0.5%的碘伏溶液浸泡术区对肾脏功能的影响.方法:采用回顾性研究的方法,收集2017年5月1日-2018年11月1日由笔者所在医院同一术者实施的髋、膝关节初次置换手术的204例病历资料,术中使用0.5%的碘伏溶液浸泡的观察组纳入108例病历资料,使用生理盐水浸泡的对照组纳入96例病历资料.观察组术中使用0.5%碘伏溶液约150 ml分别于安装假体前和关闭切口前充分浸泡术区2 min,并用0.9% 生理盐水冲洗术区至吸引出液体为清亮无色为止,对照组为使用0.9% 生理盐水于安装假体前和关闭切口前充分清洁术区至吸引出液体为清亮无色为止,并对两组患者入院时和术后24 h,48 h及7 d的肾功中的尿素及肌酐进行统计分析.结果:两组术前、术后24、48 h及7 d的尿素与肌酐值比较差异无统计学意义(P>0.05);术后24 h尿素与肌酐值较术前升高,48 h及7 d时均恢复至术前水平.观察组未发生感染,对照组发生3例感染,差异有统计学意义(P<0.05).结论:在髋、膝关节初次置换手术中使用0.5%碘伏溶液浸泡术区对肾功能无明显影响,安全性高.Objective:To evaluate the effect of 0.5%Povidone Iodine Solution immersion on renal function after joint replacement.Method:Using the method of retrospective study,the medical records of 204 cases of primary hip and knee replacement performed by the same surgeon in our hospital from May 1st,2017 to November 1st,2018 were collected.The observation group soaked with 0.5%Povidone Iodine Solution was included in the medical records of 108 cases,and the control group soaked with physiological saline was included in the medical records of 96 cases.In the observation group,about 150 ml of 0.5%Povidone Iodine Solution was used in the operation area before the prosthesis was installed and before the incision was closed for 2 minutes,and 0.9%physiological saline was used to wash the operation area until the liquid was clear and colorless.In the control group,0.9%physiological saline was used to clean the operation area before the prosthesis was installed and before the incision was closed until the liquid was clear and colorless urea and creatinine in renal function were analyzed at 24 hours,48 hours and 7 days after operation.Result:There was no significant difference in urea and creatinine between the two groups before operation and at 24 hours,48 hours and 7 days after operation(P>0.05).The urea and creatinine values at 24 hours after operation were higher than those before operation,returned to normal level at 48 hours and 7 days after operation.There was no infection in observation group,3 infection cases in control group,the difference was signifficant(P<0.05).Conclusion:The use of 0.5%Povidone Iodine Solution in the initial hip and knee replacement has no significant effect on renal function,and it is safe.
