唑来膦酸联合鲑鱼降钙素在骨质疏松性椎体压缩骨折经皮椎体成形术后的应用  被引量:2

摘  要:目的观察唑来膦酸联合鲑鱼降钙素治疗骨质疏松性椎体压缩骨折患者经皮椎体成形术后的临床效果。方法选择行经皮椎体成形术治疗的骨质疏松性椎体压缩骨折患者92例患者,随机均分为两组。对照组(46例)经皮椎体成形术后第2天起给予唑来膦酸针治疗,观察组(46例)在给予唑来膦酸针联合鲑鱼降钙素注射液治疗。对比手术前后视觉模拟评分法(VAS)评分、骨标志物(β-Crosslaps及PINP)及骨密度(BMD)。结果术后随访1年,观察组术后VAS评分及骨标志水平均低于对照组,差异均有统计学意义(P<0.01);而术后6月、1年观察组骨密度较对照显著增加(P<0.01);术后随访1年中,观察组椎体新发骨折率(6.5%)显著低于对照组(28.3%),差异有统计学意义(x^2=7.566,P<0.01)。结论唑来膦酸联合使用鲑鱼降钙素治疗骨质疏松性椎体压缩骨折经皮椎体成形术后患者疗效优于唑来膦酸单用。Objective To compare the clinical effects of zoledronic acid and salmon calcitonin in the treatment of osteoporotic vertebral compression fracture after percutaneous vertebroplasty.Methods 92 elderly patients with femoral intertrochanteric fractures for modified Evans I-III type were chosen and randomly divided into 2 groups including control group(46 patients)with zoledronic acid from 2nd day after percutaneous vertebroplasty and observation group(46 patients)with zoledronic acid and salmon calcitonin;and the visual analogue score(VAS)and the levels of bone markers(β-CrossLaps and PINP)and BMD before and after operation treatment were compared.Results In 1 year after operation with follow-up,the VAS score and bone markers of observation group were lower than control group(P<0.01).The BMD levels of observation group were significantly higher than control group in 6 months and 1 year after operation(P<0.01).In 1 year after operation with follow-up,the new fracture rate of vertebral body of observation group for 6.5%was significantly lower than control group for 28.3%(x^2=7.566,P<0.01).Conclusion Compared with Zoledronic acid alone,zoledronic acid and salmon calcitonin in the treatment of osteoporotic vertebral compression fracture after percutaneous vertebroplasty possess better clinical effects.
