慢性心力衰竭患者血清白细胞介素-9水平变化及其与心肾功能的关系

摘　　要：目的探讨慢性心力衰竭患者血清白细胞介素-9(IL-9)水平变化及其与患者心肾功能的关系。方法将2017年2月-2018年10月本院收治的60例慢性心力衰竭患者纳入研究,检测患者血清IL-9水平,分析其与患者心肾功能评价指标的关系。另选取同期在本院体检健康者60例作为对照组,比较2组血清IL-9水平。结果慢性心力衰竭组患者血清IL-9水平为(1.14±0.32)pg/ml,明显高于对照组的(0.71±0.22)pg/ml(P<0.05);NYHAⅣ级组和NYHAⅢ级组患者血清IL-9、NT-proBNP及尿微量白蛋白均高于NYHAⅡ级组(P<0.05),而肾小球滤过率均低于NYHAⅡ级组(P<0.05);NYHAⅣ级组患者血清IL-9、NT-proBNP及尿微量白蛋白均高于NYHAⅢ级组(P<0.05);LVEF<30%组患者血清IL-9、NT-proBNP、尿微量白蛋白及肾小球滤过率与LVEF>50%组和30%≤LVEF≤50%组比较差异均有统计学意义(P<0.05);肾功能正常组患者血清IL-9水平为(0.83±0.26)pg/ml,肾功能不全组患者血清IL-9水平为(1.27±0.39)pg/ml,上述差异均有统计学意义(P<0.05);血清IL-9水平与慢性心力衰竭患者NYHA心功能分级、NT-proBNP及尿微量白蛋白呈正相关(P<0.05),而与LVEF和肾小球滤过率呈负相关(P<0.05)。结论IL-9与慢性心力衰竭患者心肾功能密切相关,其在患者血清中表达上调。Objective To investigate the changes of serum interleukin-9(IL-9)levels in patients with chronic heart failure and their relationship with heart and kidney functions.Methods Sixty patients with chronic heart failure admitted to our hospital from February 2017 to October 2018 were enrolled in the study.Detection of serum IL-9 levels in patients,and the relationship between them and the evaluation index of heart and kidney functions was analyzed.In addition,60 patients who were healthy in our hospital during the same period were selected as the control group,and the serum IL-9 levels were compared between the two groups.Results The serum IL-9 level in the patients with chronic heart failure was(1.14±0.32)pg/ml,which was significantly higher than that of the control group(0.71±0.22)pg/ml(P<0.05).NYHAIV serum IL-9,NT-proBNP and urinary albumin were higher in the grade group and NYHA class III than that in the NYHAII group(P<0.05),and the glomerular filtration rate was lower than that in the NYHAII group(P<0.05).Serum IL-9,NT-proBNP and urinary albumin were higher in NYHAIV group than in NYHA class III group(P<0.05);serum IL-9,NT-proBNP,urinary albumin in patients with LVEF<30%and the glomerular filtration rate were significantly different from the LVEF>50%group and the 30%≤LVEF≤50%group(P<0.05).The serum IL-9 level in the normal renal function group was(0.83±0.26)pg/ml,serum IL-9 level in patients with renal insufficiency was(1.27±0.39)pg/ml(P<0.05).There was a positive correlation between the serum IL-9 level and the NYHA cardiac function classification,NT-proBNP and urinary albumin in patients with chronic heart rate,with the differences statistically significant(P<0.05),but it had a negative correlation with LVEF and glomerular filtration rate(P<0.05).Conclusion IL-9 is closely related to heart and kidney functions in patients with chronic heart failure,and its expression is up-regulated in the serum of patients.
