摘　　要：目的探讨miR-223和MMP-3在强直性脊柱炎(AS)患者外周血单个核细胞中的表达及其临床意义。方法选取61例AS患者作为研究组,同期健康体检者48例作为对照组,采用实时荧光定量PCR技术检测miR-223的表达,酶联免疫吸附(ELISA)技术检测MMP-3表达。通过ROC曲线分析评价miR-223和MMP-3对AS患者的诊断价值。结果患者PBMC中miR-223及血浆MMP-3的表达高于对照组,两者在活动期明显升高。结果显示,外周血单个核细胞中miR-223与血浆MMP-3、BASDAI、HLAB27、CRP及ESR存在负相关(r值分别为-0.479、-0.384、-0.538、-0.631、-0.670,P<0.05);血浆MMP-3及HLA-B27与BASDAI、CRP、ESR存在正相关相关(r值分别为0.525、0.610、0.606、0.470、0.685、0.600,P<0.05)。ROC曲线分析表明外周血单个核细胞中miR-223及血浆MMP-3对AS具有较高诊断灵敏度和特异度,联合检测能提高对疾病临床诊断能力。结论AS患者PBMC中miR-223和血浆MMP-3存在差异表达,有望成为新的临床诊断指标。Objective To investigate the clinical significance of miR-223 and MMP-3 levels in peripheral blood of patients with ankylosing spondylitis(AS).Methods A total of 61 AS patients were selected as the study group,and 48 healthy subjects were used as the control group.The miR-223 level was detected by real-time fluorescent quantitative PCR and the MMP-3 level was detected by enzyme-linked immunosorbent assay(ELISA).The diagnostic value of miR-223 and MMP-3 in patients with AS was evaluated by ROC curve analysis.Results The levels of miR-223 and MMP-3 were higher in patients than that in the controls,and increased significantly during the active period.Correlation analysis showed that there was a negative correlation between miR-223 and plasma MMP-3 in peripheral blood mononuclear cells(r=-0.479,P<0.01),and negatively correlated with BASDAI,HLA-B27,CRP and ESR(r=-0.384,r=-0.538,r=-0.631,r=-0.670,P<0.05);there was a positive correlation between plasma MMP-3 and HLA-B27 and BASDAI,CRP,ESR(r=0.525,r=0.610,r=0.606,r=0.470,r=0.685,r=0.600,P<0.01).ROC curve analysis showed that plasma miR-223 and MMP-3 have high diagnostic sensitivity and specificity for AS,and combined detection can improve the clinical diagnostic ability of the disease.Conclusion The differential levels of miR-223 and MMP-3 in the peripheral blood of patients with RA have a strong clinical diagnostic ability,are expected to become a new clinical diagnostic indicator.
