血吸虫肝病患者超声表现、诊断价值分析

摘　　要：目的:分析血吸虫肝病患者超声表现及诊断价值.方法:选取2018年1-12月在笔者所在医院就诊的疑似血吸虫肝病患者407例为研究对象.分析血吸虫肝病患者的超声影像图特征;比较超声检查、电子计算机断层扫描(CT)结果与病原学检查结果的一致性;采用ROC曲线分析超声、CT对血吸虫肝病的诊断价值.结果:血吸虫肝病的主要超声表现为肝脏表面不平整,体积与正常大小有异,存在肝脏局部肿大的现象,网状和鳞片状回声较大,肝脏回声分布不均匀,边界不清晰等.以病原学检查为标准,超声检查血吸虫肝病的Kappa值为0.729,高于CT的0.572;经ROC曲线分析得超声检查血吸虫肝病的AUC为0.858,大于CT检查的0.778(P<0.05).结论:超声对血吸虫肝病具有较高的诊断价值,且灵敏度较高.Objective:To analyze the ultrasound manifestation and diagnostic value of schistosomiasis liver disease patients.Method:A total of 407 patients with suspected schistosomiasis liver disease who were admitted in our hospital from January to December 2018 were selected as the study subjects.The characteristics of ultrasound images in patients with schistosomiasis liver disease were analyzed.The results of ultrasound examination,computed tomography(CT)were compared with the results of etiological examination.The diagnostic value of ultrasound and CT in schistosomiasis liver disease were analyzed by ROC curve.Result:The main ultrasound manifestations of schistosomiasis liver disease include uneven liver surface,different volume from the normal size,local hypertrophy of the liver,large echoes of reticular and squamous,uneven distribution of liver echo,unclear boundary,etc.The etiological examination was taken as the standard.The Kappa value of ultrasound examination in schistosomiasis liver disease was 0.729,which was higher than 0.572 of CT.The AUC of ultrasound examination of schistosomiasis liver disease was 0.858,which was higher than 0.778 of CT(P<0.05).Conclusion:Ultrasound has high diagnostic value and high sensitivity for schistosomiasis liver disease.
