摘　　要：目的:旨在探讨普通气管插管吸痰与声门下吸引吸痰在ICU人工气道患者吸痰护理中吸痰效果与安全性.方法:抽取2018年8月-2019年9月笔者所在医院入住重症监护室并行人工气道治疗患者60例进行研究,按照吸痰方案分为常规吸痰组与声门下吸痰组,每组30例.常规吸痰组采用普通气管插管方式吸痰,声门下吸痰组采用声门下吸痰法进行吸痰,统计患者吸痰过程中呛咳、呼吸困难、心率加快等并发症发生情况,同时统计两组患者日平均吸痰量、吸痰次数,以及人工气道建立时间、ICU入住时间.结果:声门下吸痰组患者吸痰期间发生呛咳、呼吸困难、心率加快、低氧血症及气道黏膜出血等并发症发生率均显著低于常规吸痰组(P<0.05);声门下吸痰组患者日平均吸痰量明显多于常规吸痰组,日平均吸痰次数、通气时间及ICU入住时间明显优于常规吸痰组,差异有统计学意义(P<0.05).结论:相比传统吸痰方式,声门下吸痰能有效缓解患者吸痰时不适症状,降低吸痰相关并发症发生率,进一步提高吸痰效果.Objective:To investigate the effect and safety of general tracheal intubation suction and subglottic suction in sucking nursing of ICU artificial airway patients.Method:A total of 60 patients in the Intensive Care Unit who underwent artificial airway treatment in our hospital from August 2018 to September 2019 were enrolled.The patients were divided into the conventional suction group and the glottic suction group according to the suction program.Each group had 30 cases.In the conventional suction group,the common tracheal intubation method was used for sucking.The subglottic suction group was sucked by the subglottic suction method.The complications such as cough,difficulty in breathing,and rapid heart rate during the sucking process were recorded.Daily average sucking volume,number of sucking,artificial airway establishment time,and ICU stay time were recorded.Result:The incidence of complications such as cough,dyspnea,increased heart rate,hypoxemia and airway mucosal bleeding were significantly lower in the glottic suction group than those in the conventional suction group(P<0.05).The average daily suction volume of the patients in the glottic suction group was significantly higher than that of the conventional suction group.The daily average number of suctions,ventilation time and ICU stay time in the glottic suction group were significantly better than those in the conventional suction group,the differences were significant(P<0.05).Conclusion:Compared with the traditional suction method,subglottic suction can effectively alleviate the symptoms of discomfort during the suction,reduce the incidence of complications related to sucking,and further improve the sucking effect.
