摘  要:目的:探讨耐甲氧西林金黄色葡萄球菌(MRSA)d 耐药性及耐药基因分型,了解其耐药基因携带的现状,以便为预防和控制MRSA的爆发性感染提供参考。方法:将2018年中国人民解放军联勤保障部队九00医院保存的50株MRSA菌株作为研究对象。使用聚合酶反应(PCR)技术对这50株MRSA菌株进行mec A基因扩增和分型,再使用纸片扩散法和肉汤稀释法对其进行药敏试验。结果:1)这50株MRSA菌株mec A基因的检出率为96%。2)这50株MRSA菌株中,SCC mec Ⅲ型菌株有4株,SCC mec Ⅳ型菌株有3株,未分型菌株有43株。3)这50株MRSA菌株对苯唑西林和β-内酰胺类药物表现出完全耐药,对多西环素、替考拉宁、利福平、复方新诺明、莫西沙星、氯霉素、左旋氧氟沙星、环丙沙星的敏感性较好,对大环内酯类药物和克林霉素的耐药性较高,对万古霉素、达托霉素和利奈唑胺均未表现出耐药。结论:该院所保存的MRSA菌株尚未出现严重的多重耐药现象,其对万古霉素、达托霉素或利奈唑胺均无耐药,其mec A基因分型有SCC mec Ⅲ型菌株、SCC mec Ⅳ型菌株等。objective:to investigate the genotypes of methicillin-resistant staphylococcus aureus(MRSA)d drug resistance and drug resistance,and to understand the status of its drug resistance gene,so as to provide reference for the prevention and control of explosive infection of MRSA.Methods:50 strains of MRSA strains were collected from the 900 hospital of the PLA joint logistic support force in 2018.The 50 strains of MRSA were amplified and genotyped by polymerase reaction(PCR),and then tested for drug sensitivity by the method of disk diffusion and broth dilution.Results:1)the detection rate of mec A gene of these 50 MRSA strains was 96%.2)in the mec A gene was detected in 48 strains of MRSA strains,SCC mecⅢtype strains have 4 strains,SCC has three strains mecⅣtype strains,not parting strains 43 strains.3)the 50 strains of MRSA strains of benzene and beta lactam drugs showed completely resistant,doxycycline,their stead ning,rifampicin and cotrimoxazole,Ming,moxifloxacin,chloramphenicol,ofloxacin,ciprofloxacin,sensitivity is good,the large ring lactone class of drugs and clindamycin resistance is higher,to vancomycin,doxycycline and rina thiazole amine were showed no resistance.Conclusion:the hospital MRSA strains has not yet been saved by severe phenomenon of multiple drug resistance to vancomycin,of azithromycin or rina thiazole amine no drug resistance,its mec genotyping has A SCC mecⅢtype s trains,SCC mecⅣtype strains,etc.
