“医院-社区-家庭”延续性护理对回肠膀胱术后造口患者生活质量及肾功能的影响

摘　　要：目的:探讨“医院-社区-家庭”延续性护理对回肠膀胱术后造口患者生活质量及肾功能的影响。方法:随机抽取2015年3月1日~2019年3月31日60例回肠膀胱术后造口患者,按随机数字表法分为研究组和对照组各30例,对照组采用常规出院护理,研究组实施“医院-社区-家庭”延续性护理,比较两组干预效果。结果:干预后,研究组中文版造口患者生活质量问卷(COH-QOL-OQ)各维度评分及总分均低于对照组(P<0.01),患者适应量表(OAI)评分高于对照组(P<0.01),肾小球滤过率高于对照组(P<0.01);研究组造口相关并发症发生率(6.67%)低于对照组(30.00%)(P<0.01)。结论:在回肠膀胱术后造口患者中应用“医院-社区-家庭”延续性护理,可改善生活质量,提升造口适应性,减轻肾功能损伤,预防造口相关并发症。Objective:To investigate the effect of"Hospital-Community-Family"continuous nursing on the quality of life and renal function of patients undergoing ileobladder surgery.Methods:From March 1,2015 to March 31,2019,60 patients with ileobladder surgery were randomly selected and divided into the study group and the control group with 30 patients in each group according to the random number table method.Results:After intervention,each dimension score and total score of City of hope-quality of life-ostomy questionnaire(COH-QOL-OQ)were lower in the study group than in the control group(P<0.01).The score of Ostomy Adjustment Inventory(OAI)was higher than that of the control group(P<0.01),and the glomerular filtration rate was higher than that of the control group(P<0.01).The incidence of stoma-related complications in the study group(6.67%)was lower than that in the control group(30.00%)(P<0.01).Conclusion:The application of"Hospital-Community-Family"continuous nursing in ileobladder operation patients can promote the quality of life,improve the adaptability of stoma,reduce the damage of renal function,and prevent the complication of stoma.
