摘　　要：目的:探讨以量化评估策略为基础的早期渐进式康复训练对机械通气患者肺功能、神经肌肉功能及预后的影响。方法:选取2017年11月1日~2019年11月30日收治的214例机械通气患者为研究对象,按照随机数字表法分为对照组和观察组各107例,对照组给予常规护理措施,观察组给予以量化评估策略为基础的早期渐进式康复训练干预;比较两组干预前后肺功能[肺活量(FVC)、峰值呼气流速(PEF)及第1秒用力呼气容积(FEV 1)]、神经肌肉功能[采用功能独立性评分量表(FIM)]及随访1、3、6个月预后情况。结果:干预后,两组FVC、PEF及FEV 1肺功能指标均优于干预前(P<0.01,P<0.05),且观察组优于对照组(P<0.01);观察组FIM评分及随访1、3、6个月预后均优于对照组(P<0.01)。结论:以量化评估策略为基础的早期渐进式康复训练,有利于机械通气患者神经肌肉功能的恢复,提高肺功能,减少并发症发生,促进患者康复进程和预后结局。Objective:To investigate the effects of early progressive rehabilitation training based on quantitative evaluation strategy on lung function,neuromuscular function and prognosis of patients with mechanical ventilation.Methods:A total of 214 patients with mechanical ventilation admitted from November 1,2017 to November 30,2019 were selected as research objects.According to the random number table method,the patients were divided into the control group and the observation group with 107 cases each.The control group was given routine nursing measures,and the observation group was given early progressive rehabilitation training intervention based on quantitative evaluation strategy.Lung function(FVC,PEF and FEV1),neuromuscular function(FIM)and follow-up was conducted for 1,3 and 6 months prognosis were compared between the two groups before and after intervention.Results:After intervention FVC,PEF,FEV1 and other lung function indexes in the two groups were better than befor(P<0.01,P<0.05),and in the observation group were better than in the control group(P<0.01),and FIM score and follow-up was conducted for 1,3 and 6 months prognosis in the observation group were better than those in the control group(P<0.01).Conclusion:The early progressive rehabilitation training based on the quantitative evaluation strategy is beneficial to the recovery of neuromuscular function in patients with mechanical ventilation,the improvement of lung function,the reduction of complications,and the promotion of patients′rehabilitation process and prognosis.
