老年危重型新冠肺炎患者成功救治1例病例报告

摘　　要：目的:还原危重型新冠肺炎患者救治临床工作的真实情景,旨在为临床救治危重型新冠肺炎患者提供诊疗、护理经验。方法:选择医疗队接诊过程中于2020年2月6日收治的危重型新冠肺炎患者1例,以病例讨论形式,还原病程记录、护理记录,特护小组人员定期组织视频会议,针对护理操作时面临的困难、操作技巧等重点、难点问题,及时进行讨论,以求获得最有效、最适合的操作方法,并对实施效果进行评价。结果:患者成功撤机拔管,脱离生命危险,各项生命体征趋于稳定,鼻咽拭子、痰新冠病毒监测阴性,于4月4日出院。结论:加强危重型患者气道、氧疗、心理等各方面细节评估和管理,落实护理人员感染防护措施,及时有效进行小组讨论、总结经验,以提高救治效果,促进患者康复。Objective:To resume the real situation of clinical healthcare provided for an elderly critically ill COVID-19 patient,so as to share our clinical treatment and nursing experience with those caring for patients severely suffering from COVID-19.Methods:To select one of the critically ill cases who hospitalized for COVID-19 since 6th February,in the form of case discussion.The records of the medical and nursing rounds conducted by the physicians and nurses were presented,so as the memos of video meetings organized by intensive care team.The main nursing procedures and skills operated were presented when facing patient′s deteriorating conditions.The measures taken were found potentially most effective and suitable for the discussed patient following retrospectively evaluating its effectiveness.Results:The patient successfully removed from the ventilator and extubated the trachea cannula,and gradually transferred to the nasal catheter for oxygen inhalation,the various vital signs tended to be stable,the nasopharyngeal swabs and sputum tests for COVID-19 were negative.The patient has been discharged from hospital on 4th April.Conclusion:Strengthen the detail assessment and management of the airway,oxygen therapy,psychology and other aspects of critically ill patients,implement the protection measures against infection of nursing staff,timely and effective group discussion and experience sharing,could potentially improve the treatment effect and promote recovery.
