国医大师熊继柏运用天麻止痉散为主治疗面肌痉挛经验

摘　　要：面肌痉挛是临床上一种常见病,常表现为一侧或双侧面部肌肉(眼轮匝肌、表情肌、口轮匝肌)反复发作的阵发性、不自主的抽搐,在情绪激动或紧张时加重,严重时可出现睁眼困难、口角歪斜以及耳内抽动样杂音,可严重影响生活。西医在治疗面肌痉挛方面效果欠佳,熊继柏教授认为面肌痉挛发生多与风邪有关,所谓风胜则动是也。治疗“面风”多从风出发,风为阳邪。高巅之上,惟风可到,故风易扰动面部经络,出现面部肌肉跳动,故常以天麻止痉散为主方加减治疗。天麻止痉散主要由天麻、全蝎、蜈蚣、僵蚕组成,主要功效为祛风止痉。熊继柏教授诊治面肌痉挛患者屡获良效,现列举熊老治疗面肌痉挛案例3则,通过对案例的分析,总结其治疗面肌痉挛疾病方面的临证经验,为临床上治疗痉挛抽动案例提供参考。Hemifacial spasm is a common clinical disease,which is often manifested as paroxysmal and involuntary spasm of one or both facial muscles(Orbicularis muscle,Expression Muscle,Basiator)repeatedly.When the emotion is excited or tense,it will aggravate.When it is serious,it may cause difficulty in opening eyes,askew of mouth angle and twitch like noise in ears,which can seriously affect life.The effect of Western Medicine on hemifacial spasm is not good.Professor Xiong Jibo thinks that the occurrence of hemifacial spasm is mostly related to wind evil,and the socalled"wind wins and movement".The treatment of"face wind"mostly starts from the wind,which is Yang evil.On the top of the mountain,only the wind can reach,so the wind easily disturbs the facial meridians,or the facial muscle beats,so Professor Xiong often takes Tianma Spasmolytic Powder as the main treatment.Tianma Spasmolytic Powder is mainly composed of Tianma,scorpion,centipede and stiff silkworm.Its main effect is to dispel wind and antispasmodic.The case of Xiong Lao treatment of facial muscle spasm is listed.Through the analysis of the case,the clinical experience of treating hemifacial spasm is summarized,which provides a reference for the clinical treatment of twitch cases.
