摘　　要：目的通过查阅文献,了解慢性非传染性疾病的流行现状,摸底成都市锦江区下属社区卫生服务中心的基础建设情况,并调取成都市锦江区社区服务中心的基础建设数据及现有慢性非传染性疾病的管理路径,探讨慢性非传染性疾病社区综合管理模式及政策性建议。方法通过定性研究,专家访谈调查、小组座谈、问卷调查等方法,与社区卫生服务中心医疗服务提供者、管理者、患者进行深度访谈,对慢病社区综合管理模式深入的探讨。结果政府对社区卫生服务中心基础建设方面投入不足,现有的慢性病管理模式符合国家的双向转诊的医疗卫生策略。结论加快推进医疗卫生公建配套,在现有社区的慢病防治体系中建立慢性疾病防治“三网”建设。Objective Through the literature,to study the status of epidemic of chronic non-communicable diseases,to find Jinjiang District infrastructure construction situation and data of community health center.The basis of the explore community chronic disease integrated management mode and policy suggestions.Methods To conduct in-depth interviews with medical service providers,managers and patients in community health service centers by means of interview survey,group discussion,questionnaire survey and other methods.Results The government did not invest enough in the community health centers.Conclusion We should accelerate the construction of public health facilities and establish the"three nets"of chronic disease prevention and control in the existing community chronic disease prevention and control system.
