导乐陪产对产妇产程进展及精神心理因素的影响

摘　　要：目的探讨导乐陪产对产妇产程进展及精神心理因素的影响。方法选取2016年8月至2018年12月我院收治的孕妇212例为研究对象,将其随机等分为对照组和观察组,对照组给予常规护理,观察组在此基础上给予导乐陪产护理,比较两组产妇自然分娩率、产程时间、视觉模拟疼痛评分(VAS)、产后出血量、汉密尔顿焦虑自评量表(HAMA)、汉密尔顿抑郁自评量表(HAMD)评分和新生儿Apgar评分。结果观察组产妇自然分娩率高于对照组(P<0.05);干预后观察组HAMA,HAMD评分均低于对照组(P<0.05);观察组第一产程、第二产程、第三产程时间均短于对照组(P<0.05);VAS评分、产后出血量均低于对照组(P<0.05),新生儿Apgar评分高于对照组(P<0.05)。结论在分娩过程中应用导乐陪产可缩短产程,降低产妇的疼痛程度,减少出血量,促进产妇自然分娩,改善产妇的焦虑、抑郁情绪,值得在临床上推广应用。Objective To explore the effect of Doula delivery for maternity progress and psychological factors.Methods To select 212 cases of pregnant women admitted to our hospital from August 2016 to December 2018 as the study subjects.They were randomly divided into a control group and an observation group.The control group was given routine nursing,and the observation group was given Doula delivery nursing to compare the natural delivery rate,duration of labor,visual analogue scale(VAS),postpartum hemorrhage,Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD)scores and newborns'Apgar scores of the two groups.Results The natural delivery rate in the observation group was higher than that in the control group(P<0.05).After intervention,the scores of HAMA and HAMD in the observation group were lower than that in the control group(P<0.05).The first stage of labor duration,the second stage of labor duration and the third stage of labor duration in the observation group were shorter than those in the control group(P<0.05).The VAS score and postpartum hemorrhage in the observation group were lower than that in the control group(P<0.05).The newborns'Apgar score in the observation group was higher than that in the control group(P<0.05).Conclusion The application of Doula delivery for maternity progress could shorten the labor process,reduce the pain of the mother,reduce the amount of bleeding,promote the mother to give birth naturally,and improve the anxiety and depression of the mother.It was worthy of clinical promotion and application.
