剖宫产术中腰硬联合麻醉的临床应用研究

摘　　要：目的研究剖宫产术中腰硬联合麻醉的临床应用效果。方法选取2017年1月至2019年9月本院收治的94名行剖宫产术产妇,采用红蓝双色球法分为对照组和观察组,各47名。对照组采用硬膜外麻醉,观察组采用腰硬联合麻醉,比较两组患者麻醉效果、麻醉情况和出生后5 min的新生儿窒息评分,统计不良反应发生率。结果观察组麻醉优良率为97.87%,高于对照组的82.98%,差异具有统计学意义(P<0.05);观察组麻醉后至新生儿娩出时间(13.16±5.28)min、麻醉起效时间(1.49±2.36)min短于对照组的(19.37±6.25)min、(3.21±3.52)min,新生儿窒息评分(9.47±0.29)分高于对照组的(9.02±0.24)分(P<0.05);观察组不良反应发生率为10.64%,低于对照组的27.66%,差异具有统计学意义(P<0.05)。结论在剖宫产术中应用腰硬联合麻醉可有效提高麻醉效果,缩短产妇麻醉时间、分娩时间,安全性良好。Objective To study the clinical application of combined spinal and epidural anesthesia in cesarean section.Methods 94 women who underwent cesarean section from January 2017 to September 2019 in our hospital were divided into two groups according to the red and blue two-color ball method,with 47 cases in each group.The control group was treated with epidural anesthesia.The observation group was treated with combined spinal and epidural anesthesia.The anesthetic effect of the two groups was compared,anesthesia and neonatal asphyxia score 5 min after birth,statistical incidence of adverse reactions.Results The excellent rate of anesthesia in the observation group(97.87%)was higher than that in the control group(82.98%),the difference was statistically significant(P<0.05).The time from anesthesia to neonatal delivery(13.16±5.28)min and anesthesia onset time(1.49±2.36)min was shorter than the control group(19.37±6.25)min,(3.21±3.52)min,the neonatal asphyxia score(9.47±0.29)scores was higher than the control group(9.02±0.24)scores(P<0.05).The incidence of reaction(10.64%)was lower than that of the control group(27.66%),the difference was statistically significant(P<0.05).Conclusion The application of combined spinal and epidural anesthesia in cesarean section can effectively improve the anesthetic effect,shorten the maternal anesthesia time and delivery time,and have good safety.
