摘  要:目的:研究电针联合康复训练对急性骨骼肌损伤模型大鼠神经肌肉接头相关蛋白nestin、Cdk5和ε-nAChR表达的影响,探讨其对骨骼肌损伤修复的可能机制。方法:清洁级SD雄性大鼠随机选取6只为正常组,余下制备右下肢的腓肠肌急性钝挫伤模型成功后,随机分为模型组与干预组,每组12只。干预组大鼠先进行跑台训练,然后选取阿是穴、承山和承筋穴位进行电针干预,每日1次,连续干预6天后休息1天。模型组和干预组分别于造模后7天、14天两个时间点分别随机选取6只大鼠进行检测。HE染色观察损伤局部骨骼肌组织病理变化;采用WB法检测nestin、Cdk5和ε-nAChR蛋白表达。结果:HE染色显示,模型组大鼠骨骼肌纤维断裂、变性、坏死,14天干预组大鼠骨骼肌细胞损伤恢复较好,瘀血和水肿明显减轻。与正常组相比,模型组大鼠各时间点nestin、Cdk5蛋白表达逐渐增加,ε-nAChR蛋白表达逐渐降低(P<0.05);与模型组比较,干预组各时间点nestin、ε-nAChR蛋白表达均增加,Cdk5蛋白表达降低(P<0.05)。结论:电针联合康复训练能通过抑制骨骼肌Cdk5蛋白表达,上调nestin、ε-nAChR蛋白表达,促进神经肌肉接头修复,从而改善大鼠骨骼肌急性损伤。Objective:To study the effect of electro-acupuncture combined with rehabilitation training on expressions of nestin,Cdk5 andε-nAChR of neuromuscular junction in rats with acute muscle injury,and to explore its possible mechanism of the therapy on the recovery of muscle injury.Methods:6 rats from 40 male SD rats were taken as the blank control,the rest rats were made into the model of acute blunt trauma of musculus gastrocnemius.The successful models were randomly divided into the model group(n=12)and the intervention group(n=12).The rats in the intervention group were given running training followed by electro-acupuncture.The points chosen for electro-acupuncture were Ashi points,Chengshan(BL57)and Chengjin(BL56).The electro-acupuncture intervention was once a day for continuous six days with one day interval.The testing was made on seventh day and fourteenth day after the rats were injured.The morphological alterations were observed by HE staining,the expressions of nestin,Cdk5 andε-nAChR were tested by Western blot method.Results:The rupture,degeneration and necrosis of skeletal muscle fibers could be seen in the model group.After 14 days of intervention,the injury improved well,and bruise and swelling were also alleviated.After modeling,the expressions of nestin and Cdk5 were significantly increased,the protein expression ofε-nAChR was decreased compared to those in the blank control group(P<0.05);after acupuncture intervention,the expression of Cdk5 were significantly decreased,the expressions of nestin andε-nAChR were greatly increased(P<0.05).Conclusion:Electro-acupuncture combined with rehabilitation training can promote the recovery of neuromuscular junction in rats with acute muscle injury.The mechanism may be related to up-regulating the expressions of nestin andε-nAChR,and down-regulating the expression of Cdk5.
