摘 要:目的总结2018-2019年冬季发病甲型流感(甲流)特点,为其诊治积累经验。方法选择2018年11月至2019年2月首都医科大学附属北京友谊医院儿科门诊确诊为甲流的患儿106例,分为甲流H1N1组(70例),甲流H3N2组(36例),同时选取同期健康体检儿童(35例)作为对照组,对比分析3组临床特征、实验室指标。结果甲流H3N2组WBC、淋巴细胞绝对值及PLT低于对照组和甲流H1N1组,差异有统计学意义(P<0.05);甲流H1N1组与对照组比较,差异无统计学意义(P>0.05)。甲流H1N1组和甲流H3N2组CD4+、CD8+、TNF-α、IL-6比较,差异无统计学意义(P>0.05),但较对照组CD4+、CD8+均显著降低,TNF-α、IL-6均显著升高,差异均有统计学意义(P<0.05);甲流H1N1组和甲流H3N2组IgG较对照组显著降低,差异有统计学意义(P<0.05);甲流H1N1组和甲流H3N2组ALT、CK-MB较对照组均显著升高,差异有统计学意义(P<0.05)。结论感染甲流H1N1的患儿血常规缺乏特异性表现,而感染甲流H3N2的患儿淋巴细胞明显降低,细胞免疫及体液免疫均参与甲流发病过程。Objective To summarize the characteristics of the onset of influenza in winter from 2018 to 2019 and to accumulate experience for the diagnosis and treatment of influenza A.Methods A stream of 106 cases was diagnosed in the Pediatric Clinic of Beijing Friendship Hospital from the beginning of November 2018 to the end of February 2019,which were divided into 70 cases of H1 N1 group,36 cases of H3 N2 group.And 36 children of concurrent physical examination were selected as a control group.The clinical characteristics and laboratory indicators of the three groups were compared and analyzed.Results The total number of leukocytes,absolute value of lymphocyte and platelet count in H3 N2 group were significantly lower than those in the control group and H1 N1 group(P<0.05).While there was no significant difference between the control group and H1 N1 group(P>0.05).There were no significant difference between the H1 N1 group and H3 N2 group in CD4+,CD8+,TNF-αand IL-6.While CD4+,CD8+in H1 N1 group and H3 N2 group were significantly lower than those in the control group,TNF-αand IL-6 were significantly higher than those in the control group(P<0.05).IgG in H1 N1 group and H3 N2 group was significantly lower than that in the control group(P<0.05).ALT and CK-MB in H1 N1 group and H3 N2 group were significantly higher than those in the control group(P<0.05).Conclusions In children with H1 N1 infection,the blood routine lacks specific performance,while the lymphocytes in children with H3 N2 infection are significantly reduced.Cellular immunity and humoral immunity are involved in the pathogenesis of influenza A.
