SLIPA喉罩全麻与气管插管全麻在小儿眼科手术的应用比较

摘　　要：目的 探讨SLIPA喉罩全麻与气管插管全麻在小儿眼科手术的应用比较.方法 选取行斜视矫正术和玻璃体切除术的患儿80例,分为Hm组(SLIPA喉罩全麻)、Qm组(气管插管全麻),观察Qm组与Hm组插管成功率;插管/置入喉罩前(T0)、插管/置入喉罩后即刻(T1)、拔管/拔喉罩后即刻(T2)HR、SBP、DBP水平,术中及术后不良反应情况.结果 Hm组喉罩插管成功率为95%,插入喉罩1次不成功患儿2例,气管插管无1次不成功患儿,Qm组麻醉成功率为100%(P>0.05),T1时间段Hm组HR、SBP、DBP指标参数明显低于Qm组(P<0.05).Qm组漏气0例、反流误吸、低氧血症例数均为0,声嘶无,咽痛5例,Hm组漏气5例,声嘶0例,咽痛0例,Hm组和Qm组总不良反应发生率相同.结论 小儿眼科手术中运用SLIPA喉罩进行全身麻醉能维持良好的通气功能,对患儿咽喉部刺激较小,血流动力学平稳,对眼压影响小,降低术后咽喉部并发症的发生率.Objective To explore the application of SLIPA laryngeal mask general anesthesia and tracheal intubation general anesthesia in pediatric ophthalmic surgery.Methods 80 cases were studied,including Hm group(SLIPA laryngeal mask general anesthesia)and Qm group(tracheal intubation general anesthesia).The success rate of intubation,T0,T1,T2,HR,SBP,DBP levels,intraoperative and postoperative adverse reactions were observed.Results The success rate of intubation was 95%in Hm group,2 cases were unsuccessful in inserting laryngeal mask once,and no cases were unsuccessful in tracheal intubation once.The success rate of anesthesia was 100%in Qm group(P>0.05).HR,SBP and DBP indexes in Hm group at T1(P<0.05).There were 0 cases of air leakage,reflux aspiration and hypoxemia in Qm group,5 cases of hoarseness,5 cases of pharyngalgia,5 cases of air leakage in Hm group,0 cases of hoarseness and 0 cases of pharyngalgia.Conclusion General anesthesia with SLIPA laryngeal mask in pediatric ophthalmic surgery can maintain good ventilation function,less irritation to the throat,stable hemodynamics and little influence on intraocular pressure.To reduce the incidence of throat complications after operation.
