房性早搏诱发阵发性心房颤动的动态心电图特征分析

摘　　要：目的 探讨房性早搏诱发阵发性心房颤动的动态心电图特征分析.?方法采用随机分层抽样法选取2016年6月~2018年6月本院房性早搏患者184例,所有患者给予动态心电图检测.结果 发生阵发性心房颤动92例,发生率为50%;心房颤动患者房性早搏前周期(815.65±80.66)ms明显长于未诱发房颤患者(700.54±70.21)ms,心房颤动患者房性早搏联律间期(331.45±30.23)ms明显短于未诱发房颤患者(500.45±51.64)ms,差异有统计学意义(P<0.05),而房性早搏前120s的房性早搏频率明显高于未诱发房颤患者,差异有统计学意义(P<0.05);92例心房颤动患者中有50例不适感,有不适感的患者平均房颤持续时间(44.21±1.23)s、房颤时平均心室率(145.65±10.64)bpm明显高于无不适感的患者(22.31±2.65)s、(112.31±12.58)bpm,差异有统计学意义(P<0.05).结论 动态心电图检测能够有效检查房性早搏患者的阵发性心房颤动,且房性早搏前周期、房性早搏联律间期和房性早搏前120s的房性早搏频率具有明显特征,心室率快,持续时间长的房颤患者不适感明显.Objective To investigate the characteristics of dynamic electrocardiogram(DCG)in patients with paroxysmal atrial fibrillation(PAF)induced by atrial premature beats.Methods 184 patients with atrial premature beats from June 2016 to June 2018 were selected by random stratified sampling,and all patients were given dynamic electrocardiogram.Results There were 92 cases of paroxysmal atrial fibrillation,the incidence was 50%;the pre atrial cycle(815.65±80.66)ms in patients with atrial fibrillation was significantly longer than that in patients without atrial fibrillation(700.54±70.21)ms,and the inter atrial period(331.45±30.23)ms in patients with atrial fibrillation was significantly shorter than that in patients without atrial fibrillation(500.45±51.64)ms(P<0.05).The frequency of atrial premature beat 120 s before premature beat was significantly higher than that of patients without induced atrial fibrillation(P<0.05),and there were 50 cases of discomfort in 92 patients with atrial fibrillation.The mean duration of atrial fibrillation(44.21±1.23)s and the mean ventricular rate(145.65±10.64)BPM during atrial fibrillation were significantly higher than those without(22.31±2.65)s and(112.31±12.58)bpm(P<0.05).Conclusion The detection of dynamic electrocardiogram can effectively detect paroxysmal atrial fibrillation in patients with atrial premature beat,and the frequency of atrial premature beat in the pre atrial cycle,the atrial premature beat interval and the 120 s before atrial premature beat has obvious characteristics.The patients with rapid ventricular rate and long duration of atrial fibrillation have obvious discomfort.
