膝关节镜下治疗髌股关节退行性变的疗效观察

摘　　要：目的 探讨膝关节镜下治疗髌股关节退行性变的疗效.方法 本研究回顾性纳入我院2017年1月~2018年1月收治的25例髌股关节退行性变患者,设为观察组,并选同期治疗的25例髌股关节退行性变患者为对照组;对照组患者25例给予关节腔注药术(关节腔注射曲安奈德针+盐酸利多卡因+玻璃酸钠针)治疗;观察组患者25例给予关节镜下(增生滑膜清理+髌骨成形+髌周去神经化+受压区微孔减压)手术治疗,比较两组疗效,对治疗前后VAS评分、Lysholm膝关节评分进行观察比较.结果 在两组不同治疗方案下,观察组膝关节镜下手术治疗有效率为96.00%(24/25),明显高于对照组关节腔注药术有效率68.00%(17/25),差异有统计学意义(P<0.05);治疗前后两组患者VAS评分、Lysholm膝关节评分差异有统计学意义(P<0.05),从治疗后组间VAS评分、Lysholm膝关节评分改善情况看,观察组膝关节镜下治疗优于高于对照组,差异有统计学意义(P<0.05).结论 ?膝关节镜下清理减压治疗髌股关节退行性变的疗效显著,可减轻患者疼痛,改善膝关节功能,值得在临床中推广应用.Objective To discuss the effect of knee arthroscopy on the degeneration of patellofemoral joint.Methods 50 patients with degeneration of patellofemoral joint treaded in our hospital from January 2017 to January 2018 were divided into observation group and control group,with 25 cases in each group.The control group received with articular cavity injection(articular cavity injection of triamcinolone acetonide needle+lidocaine hydrochloride+sodium hyaluronate needle),while the observation group received with arthroscopic surgery(hyperplasia synovial clearance+patellar formation+peripatellar denaturation+micropore decompression in compression area).VAS score and Lysholm knee score were observed and compared before and after treatment.Results In the two groups with different treatment regimentation,the effective rate of knee arthroscopic surgery in the observation group was 96.00%(24/25),which was significantly higher than that of the control group[68.00%(17/25)],and the difference was statistically significant(P<0.05).VAS score and Lysholm knee score were significantly different between the two groups before and after treatment(P<0.05).According to the improvement of VAS score and Lysholm knee score between the two groups after treatment,the effect of knee arthroscopy in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of patellofemoral joint degeneration by cleaning and decompression under knee arthroscopy has a significant effect,which can reduce the pain of patients and improve the function of knee joint.
