摘　　要：目的:了解参麦注射液的临床合理性应用情况,为临床用药提供参考。方法:调取焦作市第二人民医院2017年1月—2018年12月使用参麦注射液的患者病历1080份,并对基本情况、用药适应症、临床用药科室分布、用药疗程、用法用量、合并用药等信息进行统计分析,评价临床应用的合理性。结果:使用参麦注射液的主要为中老年患者,用药科室主要分布在心血管内科、神经内科以及呼吸科,构成比分别为30.42%、25.37%、17.50%。用药原因主要为心力衰竭和心肌梗死,占比为31.92%和25.31%。用药疗程主要分布于≤7~14 d,构成比分别为35.83%、54.82%。合并使用2~5种药物的最多,占合并用药91.00%。不合理使用180例,占16.67%,以超适应症(72.41%)为主。结论:焦作市第二人民医院参麦注射液在临床广泛应用,且存在明显的不合理用药现象,应加强对中药注射剂合理用药的干预及对医生的指导,职能部门应加大对中药注射液监管力度,保障临床合理用药。Objective:To Investigate the application of shenmai injection in the Second People's Hospital of Jaozuo city,and to provide reference for clinical rational drug use.Methods:A sample of 1100 patients using Shenmai injection from January 2017 to December 2018 were selected from the Second People's Hospital of Jiaozuo city.Statistical analysis was conducted on the basic information of patients,drug purpose,usage and dosage,course of medication,combined medication and other information to evaluate the rationality of its clinical application.Results:The number of males who used thrombus injection was significantly in elderly patients who used shenmai injection,and the drug departments were mainly neurology department、cardiology department and respiratory medicine,with composition ratios of 30.42%、25.37%and17.50%respectively.The main causes of medication were heart failure(31.92%)and myocardial infarction(25.31%).The cause of medication was mainly distributed at≤7~14 d,with composition ratios of 35.83%and54.82%,respectively.The combined use of 2 and 5 drugs accounted for 91.00%accounted for the total number of combined drug use.The number of cases of improper use was 180,accounting for 16.67%of the total cases,and the main cases were off-label indications predominate(72.41%).Conclusion:The application department of shenmai injection in the Second People's Hospital of Jiaozuo city is extensive,and there are irrational phenomena.The intervention of rational use of TCM injections and the guidance to doctors should be strengthened.Functional departments should strengthen the supervision of TCM injections to ensure rational use of clinical drugs.
