早期异位妊娠母体血清Netrin-1检测的意义分析

摘　　要：目的探讨早期异位妊娠母体血清Netrin-1检测的临床意义。方法选取拟诊早期异位妊娠患者100例作为实验组,另选取同期要求终止妊娠的宫内早孕患者100例作为对照组。采用酶联免疫吸附剂测定(ELISA)试剂检测两组患者母体血清Netrin-1表达水平。比较两组患者Netrin-1表达水平,分析Netrin-1诊断早期异位妊娠临床诊断阈值。结果实验组患者Netrin-1表达水平为(80.84±8.66)pg/ml,显著低于对照组的(353.00±9.96)pg/ml,差异有统计学意义(P<0.05)。Netrin-1浓度诊断早期异位妊娠的受试者工作特征曲线(ROC)曲线下面积的95%可信区间为(0.911,0.977)。据Youden’s index计算Netrin-1临床诊断阈值为226.50 pg/ml,即Netrin-1浓度≤226.50 pg/ml考虑早期异位妊娠可能性大。结论早期异位妊娠患者血清Netrin-1表达显著降低,考虑可能与异位妊娠患者绒毛血管形成受阻有关。Objective To investigate the significance of Netrin-1 detection in maternal serum of early ectopic pregnancy.Methods There were 100 cases of early ectopic pregnancy selected as the experimental group and another 100 cases of early intrauterine pregnancy requiring termination of pregnancy as the control group.The expression of Netrin-1 in maternal serum of the two groups was detected by enzyme-linked immunosorbent assay(ELISA).The expression level of Netrin-1 was compared between the two groups,and the clinical diagnostic threshold of Netrin-1 in the diagnosis of early ectopic pregnancy was analyzed.Results The expression level of Netrin-1 in the experimental group was(80.84±8.66)pg/ml,which was significantly lower than(353.00±9.96)pg/ml in the control group,and the difference was statistically significant(P<0.05).The 95%confidence interval of the area under the receiver operating characteristic(ROC)curve for subjects with Netrin-1 concentration in the diagnosis of early ectopic pregnancy was(0.911,0.977).According to Youden’s index,the clinical diagnosis threshold of Netrin-1 was 226.50 pg/ml,that was,the concentration of Netrin-1≤226.50 pg/ml was very likely to consider early ectopic pregnancy.Conclusion The expression of Netrin-1 in serum of patients with early ectopic pregnancy was significantly decreased,which may be related to the obstruction of villous angiogenesis in patients with ectopic pregnancy.
