摘　　要：目的了解失独(伤残)家庭签约服务供需状况,建立签约服务供需平衡模式。方法2019年1月采用目的抽样方法,对上海市浦东新区周家渡街道失独(伤残)家庭父母(需方)6人,社区卫生服务中心(供方)家庭医生和家庭医生服务管理者5人,街道失独(伤残)家庭服务机构(第三方)计生干部5人进行访谈,主要访谈内容:签约服务结构维度(人员、财力和服务)、过程维度(流程、内容和方式)、结果维度(费用控制、健康管理和便捷程度)等内容。结果需方:需求层次参差不齐,服务体验存在差异;政策了解不深,签约与否无所谓;担心隐私泄露,主动接受服务较难。供方:签约期望高于供给服务能力;因地制宜提供服务较为困难;共有与个性服务不易平衡。第三方:宣传力度不够,且对于供给内容描述过于夸大;签约服务紧密度不够;社会资源支持不足。结论构建社会多方支持体系,服务方式形式多样;加快医养服务建设步伐,满足不同层次服务需求;条块结合全面持续,予以人性化服务关怀;两个目标服务管理,履行社会职责。OBJECTIVE To establish a for signing services by surveying the situation of family contracted services for families of bereaved(disabled)their only child.Methods It applied interviews with a purpose sample of participants(6 parents of bereaved(disabled)their only child,5 family doctors and supervisor and 5 workers in family service agencies)from Zhoujiadu Street,Pudong New Area,Shanghai in January 2019 for interviewing the data about service structure dimension(people,finance,business),process dimension(process,content,method),result dimension(cost control,health management,convenience).RESULTS Demand:the service experience was different for the level of uneven demand;it was not necessary to sign for the unfamiliar contract policy;it is difficult to accept the service by worrying about privacy leakage.Supplier:the expectation on contracting services was higher than supply service capability;it was more difficult to provide services according to local conditions;common and personalized services were not easy to balance.Third parties:the propaganda was not enough,and the description of the supply content was too exaggerated;the contract service was not tight enough;the social resources were insufficiently supported.CONCLUSION It needed to build a multi-party support system in society,so that various forms of service,accelerate the pace of building medical services to meet the needs of different levels of service,provide humanized service care base on comprehensively and continuously combine the bar.
