摘  要:目的探讨在同一温度(4℃)条件下,不同时间检测FⅤ和FⅦ活性的变化,以探寻检测FⅤ和FⅦ活性的时间范围,以期为临床诊疗提供更为客观准确的实验室检查依据。方法随机抽取1:9枸橼酸钠抗凝标本(共120例,包括健康体检人群,止凝血障碍患者,非止凝血障碍其他疾病患者,不包括抗凝剂治疗患者),置于4℃环境中,分别与即时、2 h、4 h、6 h、8 h上机检测,并对结果进行统计学分析,以医学决定水平处的系统误差判定结果是否为临床接受。结果FⅤ活性测定2 h与即时结果无显著性差异(P>0.05),4 h、6 h、8 h结果低于即时,差异具有统计学意义(P<0.01),但4 h结果在医学决定水平处的系统误差小于允许误差,为临床可接受,6 h、8 h在20%、10%医学决定水平处的系统误差大于允许误差,不为临床可接受。FⅦ活性测定2 h、4 h、6 h、8 h结果明显低于即时,差异均具有统计学意义(P<0.01),其结果在医学决定水平处的系统误差大部分都大于允许误差,不为临床可接受,只有在2 h的55%、170%医学决定水平处的系统误差小于允许误差,为临床可接受。结论在低温环境时,FⅤ活性检测应在4 h内完成,而FⅦ活性应尽量即时检测,如条件有限也应在2 h内完成检测。Objective To investigate the changes of FⅤand FⅦactivity at the same temperature(4℃)at different time,in order to explore the time range of FⅤand FⅦactivity,so as to provide more objective and accurate laboratory examination basis for clinical diagnosis and treatment.Methods A total of 120 sodium citrate anticoagulant specimens(including healthy people,patients with coagulation disorder,patients with non-coagulation disorder and other diseases,excluding patients treated with anticoagulants)were randomly sampled and tested at 4~C for immediate,2 h,4 h,6 h and 8 h respectively.Statistical analysis of the results was carried out to determine whether the results were acceptable clinically based on the systematic errors at the medical decision level.Results There was no significant difference in FⅤactivity between 2 hours and immediate results(P>0.05).The results of 4 hours,6 hours and 8 hours were lower than those of immediate results(P<0.01).However,the systematic error of 4 hours was less than the allowable error at the level of medical decision,which was acceptable for clinical practice.The systematic error at 20%10%medical decision level for 6 h and 8 h is larger than the allowable error,which is not acceptable in clinical practice.The results of FⅦactivity determination in 2 h,4 h,6 h and 8 h were significantly lower than those in immediate time(P<0.01).Most of the systematic errors at medical decision level were larger than the allowable errors,which were not acceptable to clinic.Only 55%170%of the systematic errors at medical decision level in 2 H were less than the allowable errors,which were acceptable to clinic.Conclusion In low temperature environment,FⅤactivity detection should be completed within 4 hours,and FⅦactivity should be detected as soon as possible,if conditions are limited,it should also be completed within 2 hours.
