前路减压植骨内固定术治疗脊髓型颈椎病的疗效观察

摘　　要：目的:分析前路减压植骨内固定术治疗脊髓型颈椎病的疗效观察。方法:选取2015年1月-2018年1月湖北六七二中西医结合骨科医院接收50例脊髓型颈椎病患者作为观察对象,依据随机数字表法分为对照组与观察组,均为25例。对照组行自体髂骨植骨融合钛板内固定术,观察组行前路减压植骨内固定术配合中药外敷治疗。对比两组临床效果。结果:观察组、对照组术后日本骨科协会下腰痛JOA评分(Japanese Orthopaedic Association Scores,JOA)分别为(15.24±2.25)分、(11.25±2.20)分,组间差异显著(P<0.05);观察组、对照组术后神经功能缺损程度分别为(16.25±1.13)分、(19.25±1.84)分,组间对比具备统计学意义(P<0.05);观察组、对照组术后疼痛程度分别为(3.19±0.18)分、(4.63±0.46)分,组间比较具有统计学意义(P<0.05)。观察组内固松动发生率为0.0%(0/25)明显低于对照组(P<0.05);观察组100.00%(25/25)肢体功能改善率高于对照组64.00%(16/25)的肢体改善率(P<0.05)。结论:前路减压植骨内固定术治疗脊髓型颈椎病的疗效突出,术后较少出现内固定松动现象,对改善患者预后具有积极作用。Objective: To analyze the curative effect of anterior decompression and internal fixation for cervical spondylotic disease. Methods: From January 2015 to January 2018, 50 patients with cervical spondylotic spondylopathy were taken as the subjects of observation. The control group received autoiliac bone graft fusion titanium plate internal fixation and the observation group received anterior decompression intraosseous fixation with external application of traditional Chinese medicine. The clinical e ffects of 2 groups were compared. Results: The JOA score was (15.24±2.25) and (11.25±2.20) respectively in the observation group and the control group, and the difference between groups was significant (P<0.05);The levels of neurofunctional defects were (16.25±1.13) and (19.25 ± 1.84) in the observation group and the control group respectively. The comparison between the groups had statistical value (P<0.05);The pain levels were (3.19±0.18) and (4.63±0.46) in the observation group and control group respectively. The comparison between the groups was statistically significant (P<0.05). The incidence of solid loosening in the observation group was 0.0%, which was significantly lower than that in the control group (P<0.05);The improvement rate of limb function in the observation group was 100.00%(25/25), which was higher than the improvement rate of 64.00%(16/25) of the control group (P<0.05). Conclusion: Anterior decompression and internal fixation for cervical spondylotic disease is effective, which has less internal fixation and loosening after operation. It has a positive effect on the prognosis of patients.
