镂空式固定法在危重新生儿脐静脉置管护理中的应用效果

摘　　要：目的探讨镂空式固定法在危重新生儿脐静脉置管中的应用效果。方法选取100例行脐静脉置管的患儿为研究对象,采用随机数字表法将其分为观察组51例和对照组49例。观察组采用镂空式固定法固定脐静脉导管,对照组按常规采用透明敷贴密闭式固定,比较2组脐静脉导管留置时间、敷贴更换频次、脐静脉置管相关并发症的发生情况。结果观察组脐静脉导管留置时间明显长于对照组(t=21.057,P<0.001);敷贴更换频次明显少于对照组(t=-17.321,P<0.001);观察组皮肤浸渍、医用粘胶相关性皮肤损伤的发生率明显低于对照组,2组脐部感染、导管相关性血流感染的发生率比较,差异无统计学意义。结论与透明敷贴密闭式固定法相比,镂空式固定法能够延长脐静脉导管留置时间,减少敷贴更换频次,有效降低皮肤浸渍和医用粘胶相关性皮肤损伤等相关并发症的发生风险。Objective To investigate the effect of hollow-out fixation method in umbilical vein catheterization of critical neonates.Methods A total of 100 neonates undergoing umbilical vein catheterization were taken as the research object.They were divided into in the observation group(n=51)and the control group(n=49)according to the random number table method.In the observation group,the umbilical vein catheter was fixed with a hollow fixation method.In the control group,the umbilical vein catheter was routinely fixed with a transparent dressing.The umbilical vein catheter indwelling time,frequency of dressing replacement and complications of umbilical vein catheterization were compared between the two groups.Results The indwelling time of the umbilical vein catheter in the observation group was significantly longer than that in the control group(t=21.057,P<0.001).The frequency of dressing replacement was significantly less than the latter(t=-17.321,P<0.001).The incidence of related skin injuries was significantly lower than that of the control group.There was no significant difference in the incidence of umbilical infection and catheter-related bloodstream infection in the two groups.Conclusion Compared with the transparent dressing and closed fixation method,the hollow-out fixation method can prolong the indwelling time of the umbilical vein catheter,and which can reduce the frequency of dressing replacement and the complications such as skin impregnation and medical adhesive-related skin injuries.
