子午流注联合穴位贴敷治疗冠心病心绞痛的临床研究

摘　　要：目的探讨子午流注联合穴位贴敷治疗冠心病心绞痛的临床疗效。方法收集2018年12月—2019年6月期间在沈阳市中医院心病科治疗的患者100例,采用随机数表法随机分为对照组和治疗组。对照组患者进行常规的西医基础性治疗,嘱患者低盐低脂饮食,予以0.9%氯化钠注射液50 m L/5%葡萄糖注射液50 m L+硝酸甘油注射液10 mg,3~5 mL/h静脉注射,并在西医治疗的基础上予以穴位贴敷治疗。治疗组在对照组西医治疗基础上选择每日午时(11:00~13:00)予以穴位贴敷治疗,1次/d,每次时间持续20 min。两组患者均进行20 d持续性治疗。观察两组患者治疗前后中医证候积分(主症)、Hcy、NO、hs-CRP和NF-kB水平及临床有效率。结果两组患者中医证候主症评分、Hcy、NO、hs-CRP及NF-kB含量较治疗前均有所改善,且治疗后治疗组明显优于对照组,差异有统计学意义(P<0.05);治疗后,对照组总有效率为66.0%,治疗组总有效率为90.0%,差异有统计学意义(P<0.05)。结论在西医治疗基础上联合子午流注穴位贴敷治疗冠心病心绞痛能够有效地抑制炎症反应,抗血小板聚集,延缓血栓形成速率,改善胸闷、胸痛等症状,提高临床有效率。OBJECTIVE To investigate the clinical effect of the combination of meridian injection with acupoint application on coronary heart disease and angina pectoris.METHODS A total of 100 patients treated in the Department of Cardiology of Shenyang Traditional Chinese Medicine Hospital from December 2018 to June 2019 were randomly divided into control group and treatment group.Patients in the control group received conventional western medicine basic treatment.The patients were instructed to have a low-salt,low-fat diet and receive 0.9%sodium chloride injection 50 mL/5% glucose injection 50 mL+nitroglycerin injection 10 mg 3~5 m L/h intravenous Injection and acupoint application based on western medicine treatment.On the basis of western medicine treatment in the control group,the treatment group chose acupoint application at noon (11:00~13:00)daily,once a day for 20 minutes each time.Patients in both groups were treated continuously for 20 days.Observe the TCM syndrome scores(main symptoms),Hcy,NO,hs-CRP and NF-kB levels and clinical effectiveness of the two groups of patients before and after treatment.RESULTS The TCM syndrome symptom scores,Hcy,NO,hs-CRP and NF-kB contents of these two groups of patients were improved compared with those before treatment,and the treatment group was significantly better than the control group after treatment,and the differences were statistically significant(P<0.05);After treatment,the total effective rate of the control group was 66.0%,and the total effective rate of the treatment group was 90.0%,the difference was statistically significant(P<0.05).CONCLUSION Based on western medicine treatment combined with meridian flow injection acupoint application for coronary heart disease angina pectoris could effectively inhibit inflammatory response,anti-platelet aggregation,delay the rate of thrombosis,improve symptoms such as chest tightness,chest pain,and improve clinical effectiveness.
