摘　　要：目的评价止痛生肌散联合生肌象皮膏对下肢静脉痛性溃疡治疗前后伤口渗出液和组织中MMP-1、MMP-2及相关TIMPs的影响。方法应用DESIGN工具筛选出符合条件的小腿静脉痛性溃疡患者32例,收集止痛生肌散联合生肌象皮膏治疗前、后渗出液和组织,同时收集32例乳腺癌术后伤口渗出液作为急性疮面参照对象,分别采用酶联免疫吸附试验和Western blot检测渗出液和组织中MMP-1、MMP-2及相关TIMPs浓度。结果渗出液和组织中MMP-1、MMP-2、TIMP-1、TIMP-2、MMP-2/TIMP-2水平经"去腐止痛"治疗后均呈明显下降趋势,各检测指标0 d与6 d相比差异存在统计学意义(P<0.001)。治疗前后0 d、6 d渗出液与组织中MMP-1、MMP-2、TIMP-1、TIMP-2、MMP-2/TIMP-2水平均存在正相关(P<0.001)。结论止痛生肌散联合生肌象皮膏治疗下肢痛性溃疡的机制为实现慢性疮面急性化,通过降低MMP-1、MMP-2及相关TIMPs含量实现疮面愈合。Objective To assess the effect of Zhitongshengji powder(ZTSJP)combing with Shengjixiangpi plaster(SJXPP)on painful venous leg ulcers and the expression of MMP-1,MMP-2 and TIMPs in wound exudates and tissues pre-and post-treatment.Methods A total 32 cases of painful venous leg ulcer were selected by DESIGN tool.The exudates and tissues were collected pre-and post-treatment with ZTSJP combing with SJXPP,meanwhile the exudates from the wounds of 32 cases of breast cancer post-operation were collected as control group.The concentrations of MMP-1,MMP-2 and related TIMPs were detected by enzyme-linked immunosorbent assay and Western blot respectively.Results The expression levels of MMP-1,MMP-2,TIMP-1,TIMP-2 and MMP-2/TIMP-2 were decreased significantly after the treatment,and there was a statistical difference at day 0 and 6(P<0.001).There was a positive correlation among the expression of MMP-1,MMP-2,TIMP-1,TIMP-2 and MMP-2/TIMP-2(P<0.001).Conclusion The mechanism of ZTSJP combined with SJXPP in the treatment of painful venous leg ulcers by reduce the expression of MMP-1,MMP-2 and related TIMPs.
