中药汽疗联合卡泊三醇软膏治疗银屑病临床研究

摘　　要：目的:观察中药汽疗联合卡泊三醇软膏治疗银屑病的临床疗效。方法:将120例寻常型银屑病患者随机分为A、B、C 3组,每组40例;A组给予中药汽疗联合卡泊三醇软膏治疗,B组给予单纯汽疗联合卡泊三醇软膏治疗,C组单用卡泊三醇软膏治疗,疗程均为4周,观察比较各组临床疗效及不良反应,患者治疗前后银屑病皮损严重程度评分(PASI)、白细胞介素-2(IL-2)、白细胞介素-8(IL-8)、干扰素-γ(IFN-γ)、血管内皮生长因子(VEGF)水平变化。结果:总有效率A组为62.5%,B组为35.0%,C组为12.5%,3组间总有效率两两比较,差异均有统计学意义(P<0.05)。治疗后,A组PASI评分较B组、C组低,且B组较C组低,差异均有统计学意义(P<0.05)。治疗后,A组IL-2、IL-8、IFN-γ水平均较B组、C组低,且B组较C组低,差异均有统计学意义(P<0.05)。治疗后,A组VEGF水平较B组、C组低,且B组较C组低,差异均有统计学意义(P<0.05)。不良反应率A组为5.0%,B组为12.5%,C组为25.0%。A组不良反应率显著低于C组(P<0.05);A组与B组、B组与C组两两比较,差异无统计学意义(P>0.05)。结论:中药汽疗联合卡泊三醇软膏治疗银屑病疗效显著,可有效改善患者炎症状态,降低VEGF水平,且用药安全性较高。Objective:To observe the clinical effect of Chinese herbal steam therapy combined with calcipotriol ointment for psoriasis.Methods:A total of 120 cases of patients with psoriasis vulgaris were randomly divided into Group A,Group B and Group C,40 cases in each group.Group A was treated with Chinese herbal steam therapy combined with calcipotriol ointment;Group B was treated with steam therapy combined with calcipotriol ointment;Group C was simply treated with calcipotriol ointment.All groups were treated for four weeks.The clinical effect,the adverse reactions,the changes of levels of Psoriasis Area Severity Index(PASI),interleukin-2(IL-2),interleukin-8(IL-8),interferon-γ(IFN-γ)and vascular endothelial growth factor(VEGF)before and after treatment in all groups were observed and compared.Results:The total effective rate was 62.5%in Group A,35.0%in Group B and 12.5%in Group C,differences being significant(P<0.05).After treatment,the score of PASI in Group A was lower than those in the other two groups,and the one in Group B was lower than that in Group C,differences being significant(P<0.05).After treatment,the levels of IL-2,IL-8 and IFN-γin Group A were lower than those in the other two groups,and the ones in Group B were lower than those in Group C,differences being significant(P<0.05).After treatment,the level of VEGF in Group A was lower than those in the other two groups,and the one in Group B was lower than that in Group C,differences being significant(P<0.05).The incidence of adverse reactions was 5.0%in Group A,12.5%in Group B and 25.0%in Group C.The incidence of adverse reactions in Group A was significantly lower than that in Group C(P<0.05);there was no significant difference being found in the comparison of the incidence Between group A and B,between group B and C(P>0.05).Conclusion:The therapy of Chinese herbal steam therapy combined with calcipotriol ointment has significant curative effect in treating psoriasis,which can effectively improve inflammation of patients and reduce the levels of VEGF w
