家庭医生签约服务助力医药费用控制的实证探讨

摘　　要：家庭医生签约服务是实现分级诊疗的关键,也是新形势下保障和维护人民群众健康的重要途径。家庭医生团队既是人群健康的守门人,也应是人群医药费用的守门人。河北省黄骅市、上海长宁和北京方庄等地区对家庭医生控费进行探索,已初步形成一些做法并取得成效,如签约人群医药费用降低,医保基金支出节省。但由于家庭医生与医保联动不够理想,其作为医药费用守门人的职能尚未充分发挥。下一步应向家庭医生团队赋能,并健全完善相关支撑机制,使其更好地担负起医药费用守门人职能。Family doctors’contracted services is the key to achieving hierarchical diagnosis and treatment system and one vital way to safeguard people’s health in the new era.The family doctor team should be the gatekeeper both to people’s health and medi cal expenses.Localities such as Huanghua?of Hebei,Changning of Shanghai and Fangzhuang of Beijing have explored family doctors’contracted services assisting medical cost containment,and achieved positive results,e.g.reduction of medical expenses and savings of medical insurance funds.However,due to the unsatisfactory linkage between family doctors and medical insurance,its role as the gate keeper of medical expenses has not been fully met.In the future,the family doctor team should be empowered,and relevant supporting mechanism should be improved so that it can better assume the function of gatekeeper of medical expenses.
