摘　　要：目的观察加味消水圣愈汤治疗慢性心力衰竭(阳虚水瘀互结证)患者临床疗效及对NO与炎性因子水平的影响。方法将80例慢性心力衰竭(阳虚水瘀互结证)患者随机分为对照组及治疗组,各40例。对照组予西医内科综合治疗,治疗组在对照组的基础上予加味消水圣愈汤。观察并比较治疗前后2组临床疗效、中医证候积分、心功能指标、明尼苏达生活质量量表评分、血清炎性因子及NO水平,统计不良事件发生情况。结果与治疗前比较,治疗组总有效率87.17%,优于对照组的68.42%(P <0.05);治疗后治疗组中医证候积分、NYHA分级评分、明尼苏达生活质量量表评分、 LVEF、 SV、 NT-proBNP及NO水平改善程度均显著优于对照组(P <0.05);治疗后治疗组ET-1、hs-CRP、IL-6及TNF-α及水平均显著低于对照组(P <0.05)。对照组1例入院第5天因病情恶化抢救无效死亡,其他患者未发生其他不良事件。结论加味消水圣愈汤联合西医治疗慢性心力衰竭(阳虚水瘀互结证)可显著改善心功能,提高临床疗效和生活质量,并可调节ET-1、NO及降低炎症反应水平,且无不良反应发生。Objective To observe the clinical efficacy Jiawei Xiaoshui Shengyu decoction on patients with chronic heart failure(yang deficiency and water stasis syndrome) and the levels of NO and inflammatory factors. Methods Eighty patients with chronic heart failure(yang deficiency and water stasis syndrome) who were randomly divided into control group and treatment group. For example, the control group was given conventional medical treatment in western medicine, and the treatment group was given the treatment of Jiawei Xiaoshui Shengyu decoction on the basis of the control group. The clinical efficacy, TCM symptom scores, cardiac function indicators, Minnesota Quality of Life Scale scores, serum inflammatory cytokines and NO levels were observed and compared before and after treatment. Statistics on adverse events. Results Compared with before treatment, the effective rate was 87.17% in the treatment group and 68.42% in the control group(P<0.05);The TCM syndrome score, NYHA grading score, Minnesota Quality of Life Scale score, LVEF, SV, NT-proBNP and NO levels were improved after treatment, which were significantly better than the control group(P<0.05);the levels of interleukins hs-CRP, IL-6, TNF-α and ET-1 in the treatment group were significantly lower than those in the control group(P<0.05). In the control group, one patient died on the 5 th day of admission because of the deterioration of the condition, and no other adverse events occurred in the other patients. Conclusion Jiawei Xiaoshui Shengyu decoction in the treatment of chronic heart failure with yang deficiency, phlegm and blood stasis syndrome can significantly improve cardiac function, improve clinical efficacy and quality of life, and regulate ET-1, NO and reduce inflammatory response. No adverse reactions occurred.
