参芪麦味地黄汤联合二甲双胍对气阴两虚型2型糖尿病患者UACR、MODD及血流动力学的影响

摘　　要：目的:研究参芪麦味地黄汤联合二甲双胍对气阴两虚型2型糖尿病患者尿白蛋白/肌酐比值(UACR)、日间血糖平均绝对值(MODD)及血流动力学的影响。方法:选取2016年3月—2019年6月于本院接受治疗的70例2型糖尿病患者作为研究对象,按随机数字表法分为对照组与观察组,各35例。对照组在基础治疗基础上给予二甲双胍治疗,观察组在对照组基础上加用参芪麦味地黄汤治疗。比较两组患者治疗前后空腹血糖(FPG)、2 h血糖(2 h PG)、糖化血红蛋白A1c(HbA1c)、尿白蛋白/肌酐比值(UACR)、日间血糖平均绝对值(MODD)、平均血糖水平(MBG)、两室或两大血管间压力阶差(ΔP)、肺循环与体循环血流量比(QP/QS)、每博指数(SI)及心脏指数(CI)水平变化;比较两组患者临床疗效。结果:与治疗前相比,治疗后两组患者的FPG、2 h PG、HbA1c、UACR、QP/QS水平降低,ΔP、SI、CI水平升高,观察组患者的MODD水平降低(P<0.05),两组患者的MBG水平及对照组患者的MODD水平无明显变化(P>0.05);与治疗后对照组相比,治疗后观察组患者的FPG、2 h PG、HbA1c、UACR、QP/QS及MODD水平较低,ΔP、SI、CI水平及总有效率较高(P<0.05)。结论:采用参芪麦味地黄汤联合二甲双胍治疗气阴两虚型2型糖尿病患者可有效降低患者UACR、MODD水平,改善患者肾功能、血糖水平及血液循环。Objective:To study the effects of Shenqi Maiwei Dihuang Decoction combined with Metformin on urinary albumin/creatinine ratio(UACR),mean absolute value of daytime blood sugar(MODD)and hemodynamics in patients with type 2 DM patients with type of deficiency of qi and yin.Methods:70 patients met the inclusion criteria were randomly divided into the control group(n=35)and the observation group(n=35).The control group was treated with Metformin;on which basis,the observation group was also treated with Shenqi Maiwei Dihuang Decoction.The level changes of fasting blood glucose(FPG),2h blood glucose(2hPG),glycosylated hemoglobin A1c(HbA1c),UACR,MODD,mean blood glucose level(MBG),pressure gradient between two chambers or two major vessels(ΔP),the ratio of pulmonary to systemic blood flow(QP/QS),stroke volume index(SI)and cardiac index(CI)level were compared before and after the treatment in the two groups,and the clinical effects were compared between the two groups.Results:The levels of FPG,2hPG,HbA1c,UACR and QP/QS decreased,and the levels ofΔP,SI and CI increased after the treatment compared to those before the treatment in the two groups(P<0.05);the level of MODD decreased in the observation group after the treatment(P<0.05);the above changes in the observation group were more significant(P<0.05).There was no significant change in MBG level in the two groups(P>0.05).MOD level showed no significant change in the control group(P>0.05).Conclusion:Shenqi Maiwei Dihuang Decoction combined with Metfor-min can effectively reduce the levels of UACR and MODD,it can improve renal function,blood sugar level and blood circulation in patients with type 2 DM of deficiency of qi and yin.
