医院落实“两票制”政策存在问题及对策--以安徽省某二甲医院为例

摘　　要：目的旨在了解实现医药分离的"两票制"政策在二甲医院实施的现状及存在的问题,并据此提出建议。方法采用自编问卷,对安徽省5家公立二级甲等医院的医生和患者进行现场调查。调查内容包括对"两票制"的了解度及支持度、"两票制"政策分别对医患的影响、存在的问题及意见、建议等。结果 95.6%的患者表示"两票制"实施后药价下降不明显;患者自报目前就医的主要支出是药品费用;医生及患者对"两票制"支持度与了解度有差异,且差异具有统计学意义(P<0.05)。结论政府应当增加卫生投入以保证医保的报销水平;进行配套的制度改革以减小"两票制"落实的阻力;建立政府主管的售药机构以实现彻底的医药分离。Objective To know about the status quo and existing problems of the implementation of the "Two-vote System" policy for the separation of clinic from pharmacy in class 2 grade A hospitals, and to put forward suggestions. Methods A self-made questionnaire was used to conduct on-the-spot investigations of doctors and patients of five public class 2 grade A hospitals in Anhui Province. The survey included the understanding and support of the "Two-vote System" policy and the impact of the policy on doctors and patients, the problems and the suggestions regarding the policy. Results 95.6% of the patients thought that the drug price was not obviously dropped after the implementation of the policy, and that the main part of the medical expenses was the charges for drugs. There were statistically significant difference in the understanding and support of the policy between doctors and patients(P<0.05). Conclusions Government should increase health investment to ensure the reimbursement level of medical insurance, and carry out supporting system reform to reduce the resistance to the implementation of the "Two-vote System" policy, and meanwhile set up a government-sponsored drug sales agency to achieve complete separation of clinic from pharmacy.
