摘  要:目的 研究白细胞酯酶、过氧化氢、唾液酸苷酶三项联合测定在细菌性阴道炎诊断中的应用价值.方法 选取2018年1月至12月苏州大学附属常州市肿瘤医院就诊的280例疑似细菌性阴道炎女性作为观察对象展开研究,采集所有女性阴道分泌物样本,均采用Amsel法与白细胞酯酶、过氧化氢、唾液酸苷酶三项联合测定法进行检测,同时进行阴道清洁度分级检查与染色、镜检.结?果 利用Amsel法检测,阳性患者84例(30.00%),阴性患者196例(70.00%);而利用三项联合检测,阳性患者79例(28.21%),其中4例为假阳性(1.43%),阴性患者201例(71.79%),其中假阴性患者9例(3.21%).以Amsel法检测结果 为标准结果 ,三项联合检测敏感性为89.29%(75//84),阳性预测值94.94%(75/79),阴性预测值97.96%(75/79),特异性为95.52%.对照Amsel法检测结果 (χ2=0.7114,P>0.05),无统计学意义.结论 三项联合检测法在细菌性阴道炎的临床诊断中,具有较高的检出率与敏感性、特异性,操作简单、快速,检测结果 客观、稳定,可尝试取代常规Amsel法用于细菌性阴道炎的检查.Objective To study the value of leukocyte esterase,hydrogen peroxide and sialidosidase in the diagnosis of bacterial vaginitis.Methods From January to December 2018,Totally 280 women with suspected bacterial vaginitis were selected from the gynecological outpatient department of Changzhou Tumor Hospital Affiliated to Suzhou University as the objects of study.All samples of female vaginal secretions were tested by Amsel method,leukocyte esterase,hydrogen peroxide and sialidosidase.At the same time,the vaginal cleanliness was graded Examination and staining,microscopy.Results Eighty-four cases(30.00%)were positive and 196 cases(70.00%)were negative,79 cases(28.21%)were positive,4 of them were false positive(1.43%),201 of them were negative(71.79%),9 of them were false negative(3.21%).The sensitivity of the three methods was 89.29%(75/84),the positive predictive value was 94.94%(75/79),the negative predictive value was 97.96%(75/79),and the specificity was 95.52%.There was no significant difference between the two groups(χ2=0.7114,P>0.05).Conclusion In the clinical diagnosis of bacterial vaginitis,the three combined detection methods have high detection rate,sensitivity,specificity,simple and fast operation,objective and stable detection results,which can be used to replace the conventional Amsel method in the detection of bacterial vaginitis.
