超声检查在颈动脉粥样硬化中医辨证分型中的临床应用价值

摘　　要：目的:探讨超声检查在颈动脉粥样硬化(CAS)中医辨证分型中的临床应用价值。方法:将240例颈动脉粥样硬化患者作为试验对象,所有患者均开展颈动脉超声检查,同时对患者进行中医辨证分型,分为6型,对比6种分型患者的斑块性质、斑块分布部位及颈动脉狭窄率。结果:不同分型中斑块性质软斑检出率比较,差异有统计学意义(P<0.05);各证型硬斑、混合斑的检出率及颈动脉内-中膜厚度平均值(IMT)比较,差异无统计学意义(P>0.05)。各证型斑块分布部位比较,差异无统计学意义(P>0.05)。不同分型中不同程度的颈动脉狭窄率比较,差异有统计学意义(P<0.05)。结论:颈动脉超声检测指标可作为颈动脉粥样硬化患者中医辨证分型的客观指标,可进一步提高中医辨证分型的规范化,进而为临床用药及治疗提供良好的依据。Objective:To investigate the clinical application value of ultrasonography in Chinese medicine syndrome differentiation of carotid atherosclerosis(CAS).Methods:A total of 240 patients with CAS were selected as the experimental subjects.All of them were examined by carotid ultrasound and were divided into six types based on Chinese medicine syndrome differentiation.The plaque properties,plaque distribution sites and carotid stenosis rate of the 6 types of patients were compared.Results:There was significant difference in the comparison of detection rate of soft plaque between different syndrome types(P<0.05).There was no significant difference in the comparisons of detection rate of hard plaque and mixed plaque as well as carotid intimal-media thickness(IMT)(P>0.05).There was no significant difference in the comparison of plaque distribution sites(P>0.05).There was significant difference in the comparison of carotid stenosis rate of different degrees between different syndrome types(P<0.05).Conclusion:Carotid ultrasound can be used as an objective indicator of Chinese medicine syndrome differentiation in patients with CAS,which can further improve the standardization of Chinese medicine syndrome differentiation and provide a good basis for clinical medication and treatment.
