创灼膏联合按摩治疗老年卧床患者压疮临床研究

摘　　要：目的:观察在常规医护措施基础上加用创灼膏联合按摩治疗老年卧床患者压疮的临床疗效。方法:将110例蕴毒腐溃型压疮患者随机分为对照组和观察组各55例。2组均进行系统抗感染治疗,局部给予清创、换药处理。观察组以创灼膏外敷,活络油局部按摩。2组均给予改善营养状况、减轻局部压力、健康教育、局部皮肤护理等综合护理措施。2组疗程均为4周。治疗前后通过压疮愈合(PUSH)量表评定压疮愈合情况,计算疮面愈合率和记录疮面的愈合时间。比较2组的临床疗效。结果:治疗后,观察组疗效优于对照组(P<0.05)。2组PUSH量表疮面面积、疮面渗液、疮面组织类型评分和总分均较治疗前下降(P<0.01);观察组3个维度评分和总分均低于对照组(P<0.01)。观察组疮面愈合率为81.82%,高于对照组的63.64%,差异有统计学意义(P<0.05)。观察组愈合时间短于对照组(P<0.01)。结论:在常规医护措施基础上以创灼膏外敷、活络油局部按摩,可促进压疮疮面愈合,减少渗液,提高愈合质量,缩短愈合时间,临床疗效显著。Objective:To observe the clinical effect of Chuangzhuo ointment combined with massage for pressure ulcer in senile bedridden patients based on routine medical measures.Methods:Divided 110 cases of patients with pressure ulcer of ulceration due to amassed toxin type into the control group and the observation group randomly,55 cases in each group.Both groups received systematic anti-infection treatment and local debridement and dressing change.The observation group received the external application of Chuangzhuo ointment and local massage with Huoluo oil.Both groups received such comprehensive nursing measures as improving nutritional status,reducing local pressure,health education and local skin nursing.The course of two groups lasted for 4 weeks.Before and after treatment,elevated the healing condition of pressure ulcer via Pressure Ulcer Scale for Healing(PUSH),calculated the healing rate of ulcer surface,and recorded the healing time of ulcer surface.Results:After treatment,the clinical effect in the observation group was better than that in the control group(P<0.05).The scores of ulcer area,ulcer surface exudate and ulcer surface tissue type and the total score in PUSH of two groups were decreased when compared with those before treatment(P<0.01).The scores in such three dimension and the total score in the observation group were respectively lower than those in the control group(P<0.01).The healing rate of ulcer surface was 81.82%in the observation group,being higher than 63.64%in the control group,difference being significant(P<0.05).The healing time in the observation group was shorter than that in the control group(P<0.01).Conclusion:The external application of Chuangzhuo ointment combined with local massage with Huoluo oil based on routine medical measures has significant clinical effect,which can promote the healing of pressure ulcer surface,exudate of ulcer surface,improve quality of healing and shorten the healing time.
