摘　　要：目的探讨脑膜瘤MRI图像体视学参数与其良恶性之间的关系,以寻找一种临床鉴别脑膜瘤良恶性的方法。方法收集2014年5月~2018年9月我院收治的182例脑膜瘤患者的MRI图像和病理学资料,其中男性39例,女性143例,利用体视学方法测量肿瘤体积(V)及肿瘤表面积密度(Sv)。结果所有患者中,良性组共148例,平均体积36.51 cm3,平均表面积密度4.83;恶性组共34例,平均体积49.09 cm3,平均表面积密度7.06。良恶性组脑膜瘤体积比较,差异无统计学意义(P>0.05);恶性组脑膜瘤表面积密度高于良性组,差异有统计学意义(P<0.05)。恶性组Sv≥6.1827例(27/34);良性组Sv≤5.60127例(127/148)。结论MRI图像体视学方法测定脑膜瘤表面积密度可行,脑膜瘤表面积密度与病理级别有关,Sv≥6.18和Sv≤5.60可以作为鉴别脑膜瘤良恶性的指标。Objective To explore the relationship between stereological parameters of MRI images of benign and malignant meningioma, in order to find a way to clinically identify benign and malignant meningioma. Methods MRI images and pathological data of 182 meningioma patients were collected. The tumor volume(V) and tumor surface area density(Sv) were measured by stereological methods. Results There were 148 benign groups with an average volume of 36.51 cm^3 and an average surface area density of 4.83. There were 34 malignant groups with an average volume of 49.09 cm3 and an average surface area density of 7.06. There was no significant difference in the volume of meningioma between the benign and malignant groups(P>0.05). The surface area density of meningiomas in the malignant group was higher than that in the benign group and the difference between the two groups was statistically significant(P<0.05). There were 27 cases(27/34) in the malignant group Sv≥6.18 and 127 cases(127/148) in the benign group Sv≤5.60. Conclusions The method of stereology to determine the surface area density of meningioma is feasible. The surface area density of meningioma is related to the pathological grade. Sv≥6.18 and Sv≤5.60 can be used as indicators to identify the benign and malignant meningioma.
