862例正常妊娠期孕妇心电图异常研究

摘　　要：目的探讨862例正常妊娠期孕妇心电图的异常情况。方法回顾性分析了2018年4月-2019年3月在本院实行产科检查的862例妊娠期孕妇心电图情况,对其异常心电图的产生率与病因作进一步的分析。结果观察结果显示239例妊娠期患者发生心电图异常,占比27.73%(239/862)。发生占比最高的为窦性心动过速,短P-R间期、T波改变次之。其中左室高电压5例(0.580%,5/862),完全性右束支传导阻滞2例(0.232%,2/862),Ⅰ度房室传导阻滞1例(0.116%,1/862),Ⅲ度房室传导阻滞1例(0.116%,1/862),房室交界处期前收缩2例(0.232%,2/862),房性期前收缩8例(0.928%,8/862),窦性心动过缓者8例(0.928%,8/862),ST改变8例(0.928%,8/862),室性期前收缩11例(1.28%,11/862),T波改变16例(1.86%,16/862),短P-R间期34例(3.94%,34/862),窦性心动过速143例(16.59%,143/862);20~30岁区间患者心电图异常发生率最高的为窦性心动过速(31.80%),其次为短P-R间期(9.20%),31~40岁区间患者心电图异常发生率最高的为窦性心动过速(24.69%),其次为短P-R间期(4.18%),T波改变(2.51%)。结论妊娠期间心电图异常的产生率极高,其中最为多见的是窦性心动过速,然后就是短P-R间期、以及T波改变,室性期前收缩、房性期前收缩与ST段改变也相对较多见。Objective To investigate the abnormalities of electrocardiogram in 862 pregnant women during normal pregnancy.Methods Retrospectively analyzed the electrocardiogram of 862 pregnant women who underwent obstetric examination in our hospital from April 2018 to March 2019,and further analyzed the abnormal ECG generation rate and etiology.Results Observations showed that 239 cases of gestational age patients had ECG abnormalities,accounting for 27.73%(239/862).The highest proportion of sinus tachycardia occurred,followed by short P-R interval and T wave change.There were 5 cases of high left ventricle(0.580%,5/862),2 cases of complete right bundle branch block(0.232%,2/862),and 1 case of atrioventricular block(0.116%,1/862),1 case of grade III atrioventricular block(0.116%,1/862),2 cases of premature contraction at the atrioventricular junction(0.232%,2/862),and 8 cases of atrial premature contraction(0.928%,8/862),8 patients(0.928%,8/862)with sinus bradycardia,8 patients(0.928%,8/862)with ST change,and 11 patients with ventricular premature contraction(1.28%,11/862),T wave changes in 16 cases(1.86%,16/862),short P-R interval in 34 cases(3.94%,34/862),sinus tachycardia in 143 cases(16.59%,143/862);20-30 years old the highest rate of abnormal electrocardiogram in patients with interval was sinus tachycardia(31.80%),followed by short P-R interval(9.20%).The highest incidence of abnormal electrocardiogram in patients aged 31-40 years was sinus tachycardia(24.69%),followed by a short P-R interval(4.18%)and a T wave change(2.51%).Conclusion During pregnancy,the incidence of abnormal ECG is extremely high,the most common of which is sinus tachycardia,followed by short P-R interval,and T wave changes,ventricular premature contraction,atrial premature contraction and ST segment changes.Also relatively more common.
