摘　　要：目的:探讨阴式引导下清宫术治疗Ⅱ型剖宫产瘢痕妊娠(cesarean scar pregnancy,CSP)的效果及出血风险。方法:选取2016年3月-2019年3月本院收治的Ⅱ型CSP患者102例为对象,根据患者自愿原则,分为选择阴式引导下清宫术56例(观察组),直接行阴式妊娠物清除及子宫修补术46例(对照组),比较两组手术时间、出血量。结果:两组无一例并发症及改行经腹手术,术中出血量及手术时间观察组均低于对照组(P<0.05),所有患者术后恢复好。结论:针对Ⅱ型CSP患者采用阴式引导下清宫术,安全可靠、创伤小、恢复快,具有较高的临床应用价值。Objective To investigate the effects of uterus curettage under guidance by vaginal surgeryfor typeⅡcesareanscar pregnancy(CSP)and the risk of bleeding.Methods A total of 172 patients with typeⅡCSP who were admitted to our hospital during the period from march 2016 to march 2019 were selected as research.According to the principle of voluntary,56 patients choose uterus curettage under guidance by vaginal surgery,46 patients firstly choose routine removal of pregnancy material and hysterorrhaphy by vaginal surgery.The operation effect,operation time,blood loss,hospital stay,cost,and postoperative recovery weresummarized.Results 43 patients were only underwent uterus curettage by guidance,13 patients received routine removal of pregnancy material and hysterorrhaphy in time due to intraoperative hemorrhage or muscle layer weakness,and none of the 13 patients had complications or conversion of transabdominal surgery,the intraoperative blood loss and operative timewere lower than control group(P<0.05).All patients recovered well after surgery.Conclusion The patients with typeⅡCSP were treated byuterus curettageunder guidance of vaginal surgery,this treatment is safe and reliable,small trauma,rapid recovery,and has a good clinical application value..
