腹腔镜联合胆管镜取石治疗胆管结石的临床效果研究

摘　　要：目的探究腹腔镜联合胆管镜取石治疗胆管结石的临床效果.方法选取江苏省启东市第三人民医院2013年6月至2018年5月收治的80例胆管结石患者,按照入院顺序分为对照组和观察组,每组40例.对照组给予传统开腹和胆道镜治疗,观察组给予腹腔镜联合胆道镜治疗,记录两组患者手术时间、术中出血量、排气时间和住院时间,比较临床治疗效果.结果观察组患者手术时间相较于对照组更长,但是术中出血量、排气时间和住院时间更短,差异有统计学意义(P<0.05);观察组患者伤口感染发生率为0,明显低于对照组的10.00%,差异有统计学意义(P<0.05).结论将腹腔镜联合胆管镜取石治疗用于胆管结石患者的治疗过程中,效果显著,可在一定程度上减少术中出血量,缩短排气时间和住院时间,降低术后伤口感染发生概率.Objective To explore the clinical effect of laparoscopy combined with choledochoscopy in the treatment of bile duct stones.Methods A total of 80 patients with cholelithiasis admitted to the third people's Hospital of Qidong,Jiangsu Province from June 2013 to May 2018 were divided into control group and observation group according to the order of admission,with 40 cases each group.The control group was treated with traditional laparotomy and choledochoscopy.The observation group was treated with laparoscopy combined with choledochoscopy.The operation time,intraoperative blood loss,exhaust time and hospitalization time of the two groups were recorded,and the clinical treatment was compared Healing effect.Results Compared with the control group,the operation time of the observation group was longer,but the bleeding volume,exhaust time and hospitalization time were shorter,the difference was statistically significant(P<0.05);the incidence of wound infection in the observation group was 0,significantly lower than 10.00%of the control group,the difference was statistically significant(P<0.05).Conclusion laparoscopic combined with choledochoscopic lithotripsy is effective in the treatment of choledocholithiasis.It can reduce the amount of intraoperative bleeding,exhaust time and hospitalization time to a certain extent,and reduce the incidence of postoperative wound infection.
