摘　　要：目的探讨通道下经椎间孔入路减压腰椎椎间融合联合经皮螺钉内固定治疗退行性腰椎疾病应用体会.方法选取张家港市第一人民医院2017年8月至2019年5月收治的40例退行性腰椎疾病患者为研究对象,根据患者入院ID号将其平分为观察组与对照组,每组各20例.观察组采用通道下经椎间孔入路减压腰椎椎间融合联合经皮螺钉内固定治疗,对照组采用传统后路开放手术治疗,比较两组手术指标、功能障碍评分及疼痛评分.结果观察组手术用时、下床活动时间及住院用时比对照组短,术中出血量少于对照组(P<0.05);术前两组患者Oswestry功能障碍指数(ODI)及面部表情疼痛量表(FPS)评分组间比较差异无统计学意义(P>0.05),术后两组ODI及FPS评分均得以降低,且观察组下降幅度高于对照组(P<0.05).结论针对退行性腰椎疾病患者,采用通道下经椎间孔入路减压腰椎椎间融合联合经皮螺钉内固定治疗能缩短手术用时、术中出血量、下床活动时间及住院用时,有效改善患者功能障碍,缓解疼痛.Objective To explore the experience of the treatment of degenerative lumbar diseases by decompression and lumbar interbody fusion combined with percutaneous screw internal fixation.Methods Totally 40 patients with degenerative lumbar diseases admitted to Zhangjiagang First People's Hospital from August 2017 to May 2019 were selected as the subjects.According to the ID number of patients admitted to the hospital,they were divided into two groups,the observation group and the control group,with 20 cases each group.The observation group was treated by decompression and fusion of lumbar vertebrae through intervertebral foramen and percutaneous screw fixation.The control group was treated by traditional posterior open surgery.The operation indexes,dysfunction scores and pain scores of the two groups were compared.Results The operation time,ambulation time and hospitalization time in the observation group were shorter than those in the control group,and the amount of bleeding in the operation was less than that in the control group(P<0.05).There was no significant difference in ODI and FPS between the two groups(P>0.05).After the operation,the scores of ODI and FPs in the two groups were decreased,and the decrease in the observation group was larger than that in the control group(P<0.05).Conclusion For the patients with degenerative lumbar diseases,the treatment of decompression through the intervertebral foramen and lumbar interbody fusion combined with percutaneous screw fixation can shorten the operation time,intraoperative hemorrhage,the time of getting out of bed and hospitalization,effectively improve patients'dysfunction and relieve pain.
