摘  要:目的分析将椎弓根钉棒系统联合自体颗粒骨椎间打压植骨的治疗方法应用于退变性腰椎管狭窄症的临床治疗效果.方法选取中国人民解放军东部战区总医院2017年9月至2018年9月进行治疗退变性腰椎管狭窄症的42例患者,根据入院时间分为S组和D组,每组21例.D组实施单纯椎板减压自体颗粒骨椎间打压植骨法进行治疗,S组在D组的基础上融合椎弓根钉棒系统进行治疗.比较两组患者围术期的相关指标、术后的腰椎功能评分、腰痛评分以及椎间隙距离.结果S组患者手术时间、术中出血量以及术后引流量均少于D组,差异有统计学意义(P<0.05);S组患者术后的腰椎功能评分、腰痛评分以及椎间隙距离均优于D组患者,差异有统计学意义(P<0.05).结论将椎弓根钉棒系统联合单纯自体颗粒骨椎间打压植骨的治疗方法应用于退变性腰椎管狭窄症的临床治疗显著,不仅可以恢复患者的椎间隙距离,提升患者的腰椎功能,还可以改善患者腰痛的情况.Objective To analyze the clinical effect of pedicle screw-rod system combined with autogenous granular bone intervertebral compression bone grafting in the treatment of degenerative lumbar spinal stenosis.Methods A total of 42 patients with degenerative lumbar spinal stenosis treated in General Hospital of the East War Zone of PLA from September 2017 to September 2018 were divided into group S and group D according to admission time,21 cases per group.Group D was treated with autogenous granular bone intervertebral compression bone grafting alone.Group S was treated with pedicle screw-rod system on the basis of group D.The perioperative indexes,lumbar function score,low back pain score and intervertebral space distance were compared between the two groups.Results The operation time,intraoperative blood loss and postoperative drainage volume of group S were less than those of group D,and the difference of data was statistically significant(P<0.05).The scores of lumbar function,low back pain and intervertebral space of group S were better than those of group D,and the difference of data was statistically significant(P<0.05).Conclusion The pedicle screw-rod system combined with autogenous granular bone intervertebral compression bone grafting was effective in the treatment of degenerative lumbar spinal stenosis.It can not only restore the intervertebral space distance of patients and improve the lumbar function of patients,but also relieve the low back pain.
