摘　　要：目的探究后路病灶清除植骨融合内固定术对脊柱结核患者血沉(ESR)、C反应蛋白(CRP)的影响。方法对淮安解放军第82医院2014年1月至2018年12月收治的40例脊柱结核患者进行回顾性分析,按手术方法将其分为对照组与观察组,对照组行后路内固定前路病灶清除植骨融合术,观察组行后路病灶清除植骨融合内固定术。持续观察30 d。统计2组患者的各项手术指标及治疗效果;比较2组患者手术前及术后7 d ESR值、CRP水平以及Cobb角变化和疼痛情况。结果手术后对比,观察组手术时间、术中出血量以及植骨融合时间均短于或少于对照组(P<0.05)。与手术前相比,术后7 d 2组ESR值、CRP水平均明显降低,且观察组明显低于对照组(P<0.05)。与手术前相比,术后7 d 2组Cobb角平均值均降低,且观察组小于对照组(P<0.05);术后7 d 2组VAS评分均下降,且观察组低于对照组(P<0.05)。结论给予脊柱结核患者后入路病灶清除植骨融合内固定术不仅能缩短患者手术时间,降低手术中出血量及疼痛,还能明显改善其病灶活动情况与后凸畸形矫正情况,促进患者康复。Objective To explore the effect of posterior approach debridement and fusion on ESR and CRP in patients with spinal tuberculosis.Methods Retrospective analysis was performed on 40 patients with spinal tuberculosis admitted to the 82nd hospital of huai'an people's liberation army from January 2014 to December 2018.They were divided into control group and observation group according to surgical methods.Keep looking for 30 days.The surgical indexes and therapeutic effect of patients in 2 groups were counted.ESR value,CRP level,Cobb Angle changes and pain at 7 d before and after surgery were compared between the two groups.Results Compared with the control group,the operation time,intraoperative hemorrhage and bone graft fusion time in the observation group were shorter or shorter(P<0.05).The ESR and CRP levels of the two groups were significantly lower than those of the control group(P<0.05).Compared with that before operation,the average value of Cobb angle in the observation group was lower than that in the control group(P<0.05),and the VAS score in the observation group was lower than that in the control group(P<0.05).Conclusion In addition to shortening the operation time,reducing the amount of bleeding and pain during the operation,it can also significantly improve the activity of the lesions and the correction of kyphosis,and promote the recovery of the patients.
