自拟清气凉营汤鼻饲联合抗生素对急诊呼吸机相关性肺炎(气营两燔证)患者CRP、PCT的影响

摘　　要：目的 研究自拟清气凉营汤鼻饲联合抗生素对急诊呼吸机相关性肺炎(气营两燔证)患者C反应蛋白(CRP)、降钙素原(PCT)的影响.方法 回顾性分析齐齐哈尔市中医医院2017年5月至2018年5月收治的急诊呼吸机相关性肺炎(气营两燔证)患者70例,按治疗方式不同分为对照组与观察组,各35例.对照组给予常规药物抗生素治疗,观察组在对照组基础上加用自拟清气凉营汤鼻饲,2组患者均治疗2周.观察治疗前后2组中医症候积分、肺功能指标及血清PCT、CRP水平变化.结果 与治疗前比,治疗后2组中医证候积分均降低,且观察组低于对照组(P<0.05);肺功能参数最大自主分钟通气量(MVV)、肺活量(VC)、1秒用力呼气容积(FEV1)水平均升高,且观察组高于对照组(P<0.05);血清CRP,PCT水平均下降,且观察组低于对照组(P<0.05).结论 自拟清气凉营汤鼻饲联合抗生素治疗急诊呼吸机相关性肺炎(气营两燔证)可有效改善患者症状,调节炎症因子水平,抑制机体炎症反应,改善肺功能.Objective To study the effect of self-made Qingqi Liangying Decoction combined with nasal feeding and antibiotics on CRP and PCT in patients with emergency ventilator-associated pneumonia(Qiying two syndromes).Methods A retrospective analysis was conducted on 70 cases of emergency ventilator related pneumonia(qi-ying two burnt syndromes)admitted to qiqihar traditional Chinese medicine hospital from May 2017 to May 2018.According to different treatment methods,the patients were divided into control group and observation group,35 cases each.The control group was treated with conventional antibiotics,and the observation group was treated with self-designed qingqi liangying decoction for nasal feeding on the basis of the control group.Both groups were treated for 2 weeks.The changes of TCM syndrome score,lung function index,serum PCT and CRP level in the two groups before and after treatment were observed.Results Compared with before treatment,the TCM syndrome scores of the two groups decreased after treatment,the observation group was lower than the control group(P<0.05);the levels of pulmonary function parameters MVV,VC,FEV1 increased,and the observation group was higher than the control group(P<0.05);Serum CRP and PCT levels decreased,the observation group was lower than the control group(P<0.05).Conclusion Self-designed Qingqi Liangying Decoction combined with nasal feeding and antibiotics for emergency ventilator-associated pneumonia(Qiying two syndromes)can effectively improve the symptoms of patients,regulate the level of inflammatory factors,inhibit the body's inflammatory response,and improve lung function.
