摘　　要：目的 探究复方右旋糖酐40注射液对失血性休克患者血压及休克指数(SI)、血红细胞比容(HCT)的影响.方法 回顾性分析2019年5月至8月新疆医科大学第一附属医院收治的失血性休克患者84例,按治疗方式不同分为实验组与对照组,各42例.实验组给予复方右旋糖酐40注射液治疗;对照组给予平衡盐溶液治疗,2组均滴注30min.比较2组治疗前、治疗后30min的心律(HR)收缩压(SBP)舒张压(DBP)、SI、HCT;比较2组治疗后30min临床疗效、不良反应发生情况.结果 治疗后30min观察组总有效率(97.63%)高于对照组(73.81%)(P<0.05);与治疗前相比,治疗后30min2组HR、SI均降低,且观察组低于对照组;2组SBP、HCT、DBP均上升,且观察组高于对照组(P<0.05);治疗后30min实验组不良反应总发生率(4.76%)高于对照组(2.38%),但组间差异无统计学意义(P> 0.05).结论 复方右旋糖酐40注射液可有效改善失血性休克患者血压、SI及HCT水平,纠正休克,疗效显著.Objective To investigate the effect of compound dextran 40 injection on blood pressure,SI,and HCT in patients with hemorrhagic shock.Methods A total of 84 patients with hemorrhagic shock treated in our hospital from May 2019 to August 2019 were retrospectively analyzed.They were divided into experimental group and control group according to different treatment methods,42 cases each.The experimental group was treated with compound dextran 40 injection;The control group was treated with balanced salt solution,and the two groups were treated for 30 min.The HR,SBP,DBP,SI,and HCT of the two groups were compared before and 30 minutes after treatment;The clinical efficacy and adverse reactions of the two groups were compared 30 minutes after treatment.Results The total effective rate(97.63%)in the experimental group at 30 minutes after treatment was higher than that in the control group(73.81%)(P<0.05);Compared with before treatment,the HR and SI reduced in both groups at 30 minutes after treatment,and the experimental group was lower;SBP,HCT,and DBP increased in both groups,and the experimental group was higher than the control group(P<0.05);the total incidence of adverse reactions in the experimental group(4.76%)30 minutes after treatment was higher than that in the control group(2.38%)(P>0.05).Conclusion Compound dextran 40 injection can effectively improve blood pressure,SI,HCT level and correct shock in patients with hemorrhagic shock,the effect is significant.
