摘  要:目的观察急性局灶性脑缺血再灌注大鼠皮质神经元超微结构变化及苦碟子注射液对此变化的保护作用。方法将150只雄性Wistar大鼠随机分为假手术组、模型组和治疗组[腹腔注射苦碟子注射液8.4 g/(kg·d),1次/d],每组42只,另24只备用。后2组采用线栓法制备左侧大脑中动脉闭塞1 h再灌注大鼠模型。3组于造模后1、3、6、24、72 h和7 d取材,每个时间点7只,分别于24、72 h和7 d采用Longa等评分法进行神经行为学评分;于1、3、6、24、72 h和7 d采用苏木精-伊红染色观察皮质神经元的组织病理学变化,采用透射电镜观察皮质神经元的超微结构变化。结果模型组和治疗组24、72 h和7 d神经行为学评分较假手术组显著升高[(2.40±0.54)分和(1.60±0.54)分vs 0分,(2.70±0.75)分和(1.40±0.54)分vs 0分,(2.30±0.49)分和(1.30±0.49)分vs 0分,P<0.05],而且治疗组上述3个时间点神经行为学评分较模型组明显减低(P<0.05)。与假手术组比较,模型组大鼠1、3、6、24、72 h和7 d的病理形态学及皮质神经元超微结构损伤严重,神经元细胞及细胞器形态明显呈缺血改变,细胞器数量减少;与模型组比较,治疗组大鼠1、3、6、24、72 h和7 d的病理形态学及皮质神经元超微结构的损伤明显减轻。结论苦碟子注射液能明显改善局灶性脑缺血再灌注大鼠神经功能缺损症状,并对缺血再灌注损伤后大鼠脑皮质神经元具有明显保护作用。Objective To study the effect of Kudiezi(Ixeris Sonchifolia)Injection on ultrastructure of cortical neurons in rats with acute focal cerebral ischemia following reperfusion.Methods One hundred and fifty male Wistar rats were randomly divided into sham operation group,model group and Kudiezi Injection treatment group(42 in each group)with 24 reserved for use.An ischemia reperfusion model of rats was established by Longa’s occlusion at 1 h after left middle cerebral artery occlusion.The neurological behaviors were scored with the Longa scoring system at 24 and 72 h and on day 7 after acute ischemia reperfusion.The histopathological changes of cortical neurons and the ultrastructural changes of cortical neurons were observed by transmission electron microscopy with hematooxylin-erosin staining at 1,3,6,24,72 h and on day 7 after acute ischemia reperfusion.Results The neurological behavior score was 0 in sham operation group and was significantly higher in model group and Kudiezi Injection treatment group than in sham operation group at 24 and 72 h and on day 7 after acute ischemia reperfusion(2.40±0.54 and 1.60±0.54 vs 0,2.70±0.75 and 1.40±0.54 vs 0,2.30±0.49 and 1.30±0.49 vs 0,P<0.05)and was significantly lower in Kudiezi Injection treatment group than in model group at 24 and 72 h and on day 7 after acute ischemia reperfusion(P<0.05).The change of pathological morphology,the ultrastructure injury of cortical neurons,the ischemic change of neuronal cells and cell organ morphology were significantly severer,the number of cell organs was significantly smaller in model group than in sham operation group while the change of pathological morphology and ultrastructure of neurons was significantly milder in Kudiezi Injection treatment group than in model group at 1,3,6,24,72 h and on day 7 after acute ischemia reperfusion.Conclusion Kudiezi Injection can effectively improve the neurological deficit symptoms of rats with focal cerebral ischemia following reperfusion and protect rats against injury of neuronal cells
